【摘要】：截止2016年初,我國60周歲及以上人口占13.26%,與2000年人口普查數(shù)據(jù)相比增加了2.93%。隨著出生率降低、人們的預(yù)期壽命的延長,我國老齡化問題隨之而來。有關(guān)醫(yī)學(xué)統(tǒng)計(jì)分析表明60周歲及以上人口是癌癥高發(fā)病率人群。目前中國老年癌癥患者正在呈逐年遞增趨勢。由于年齡的增長和身體機(jī)能的不斷衰退,老年癌初患者的心理上更加脆弱。因癌癥的不可逆性,國內(nèi)外面對(duì)老年癌初患者最普遍的方法是醫(yī)學(xué)治療和臨床照顧相結(jié)合。近年來以心理學(xué)興起為背景的心理治療和臨床呵護(hù)在緩解老年癌初患者心理焦慮的服務(wù)中不斷進(jìn)步。借鑒前人在緩解老年癌初患者心理焦慮的理論基礎(chǔ)和實(shí)踐經(jīng)驗(yàn),同時(shí)結(jié)合筆者在吉大一院放療科實(shí)習(xí)的相關(guān)經(jīng)歷,筆者試圖從社會(huì)工作的角度,把老年社會(huì)工作的相關(guān)理論和社會(huì)工作的方法相結(jié)合,把理性情緒療法用于緩解老年癌初患者心理焦慮的個(gè)案工作中去,探索社會(huì)工作在其中的實(shí)踐意義。筆者試圖以系統(tǒng)理論著眼案主整體情況,運(yùn)用理性情緒療法緩解案主的焦慮情緒,以此來檢測社會(huì)工作在老年癌初患者心理焦慮服務(wù)中的作用。在項(xiàng)目進(jìn)行階段,筆者以系統(tǒng)視角理論作為開展個(gè)案工作的主要理論,運(yùn)用理性情緒療法和個(gè)案工作方法相融合的社工形式,通過8次個(gè)案服務(wù)面對(duì)面和案主進(jìn)行交流,同時(shí)聯(lián)系案主家庭相關(guān)成員,了解其社會(huì)關(guān)系網(wǎng)絡(luò),引導(dǎo)案主發(fā)現(xiàn)問題并共同制定服務(wù)計(jì)劃和所要達(dá)到的服務(wù)目標(biāo),和案主親人共同見證案主的成長與進(jìn)步。個(gè)案服務(wù)結(jié)束后,筆者通過焦慮自評(píng)量表(SAS)來對(duì)比案主的心理變化并根據(jù)其家人的交流和督導(dǎo)的評(píng)價(jià)來獲得服務(wù)的最后結(jié)果。以理性情緒療法來緩解老年癌初患者心理焦慮,通過面對(duì)面的溝通引導(dǎo)案主發(fā)現(xiàn)問題,培養(yǎng)案主發(fā)現(xiàn)問題和解決問題的能力,發(fā)掘服務(wù)對(duì)象自身潛能,整合案主相關(guān)資源來解決服務(wù)對(duì)象的焦慮情緒,達(dá)到案主外在行為的改變。
[Abstract]:By the beginning of 2016, the population aged 60 and above accounted for 13.26 percent, an increase of 2.93 percent compared with the 2000 census data.With the decrease of birth rate and the prolongation of life expectancy, the aging problem of our country follows.Medical statistics and analysis show that the population aged 60 and above is a high incidence of cancer.At present, the elderly cancer patients in China are increasing year by year.As a result of the age and the body function receding, the old age cancer early patient psychologically is more vulnerable.Because of the irreversibility of cancer, medical treatment and clinical care are the most common methods to face the aged patients with cancer at home and abroad.In recent years, psychotherapy and clinical care, with the background of the rise of psychology, have made continuous progress in the service of relieving the psychological anxiety of the aged patients at the beginning of cancer.Referring to the theoretical basis and practical experience of relieving the psychological anxiety of the aged patients at the beginning of cancer, and combining with the relevant experience of the author's practice in the radiotherapy department of the first Hospital of Jida, the author tries to make an attempt from the angle of social work.Combining the theory of social work with the method of social work, the rational emotional therapy is used to alleviate the psychological anxiety of the elderly patients with cancer, and the practical significance of social work is explored.The author attempts to use the system theory to focus on the overall situation of the victim and to use rational emotional therapy to alleviate the anxiety of the victim so as to examine the role of social work in the psychological anxiety service of the elderly patients with cancer at the beginning of the year.In the stage of the project, the author takes the theory of system perspective as the main theory to carry out the case work, using the social worker form of rational emotional therapy and the method of case work, through 8 times of case service face-to-face and the case owner to communicate.At the same time, contact the relative members of the victim's family, understand its social network, guide the owner to find out the problem and make the service plan and the service goal to be achieved together, and the relatives of the victim witness the growth and progress of the victim.After the case service, the author compares the psychological changes of the victim with SAS and obtains the final results according to the communication and supervision of his family.Rational emotional therapy was used to relieve the psychological anxiety of the aged patients at the beginning of cancer, to guide the victim to discover problems through face-to-face communication, to cultivate the ability of finding problems and solving problems, and to explore the potential of the clients themselves.Integrate the relevant resources to solve the anxiety of the client and achieve the change of the external behavior of the client.
