醫(yī)患溝通中的個(gè)案工作研究

【摘要】:2015年頒布的《中國(guó)醫(yī)師執(zhí)業(yè)狀況白皮書》顯示,傷醫(yī)事件數(shù)量逐步增加。這表明了醫(yī)患關(guān)系正在惡化。筆者有幸成為一名醫(yī)務(wù)社會(huì)工作志愿者,借此機(jī)會(huì)了解到醫(yī)患關(guān)系緊張的現(xiàn)狀。隨著人們?cè)絹?lái)越關(guān)注醫(yī)療服務(wù),人們開(kāi)始意識(shí)到,要從根本上預(yù)防和治療疾病,必須考慮和關(guān)注患者的社會(huì)和心理因素。社會(huì)工作在這方面擁有其他無(wú)可比擬的優(yōu)勢(shì),在介入醫(yī)患關(guān)系時(shí),醫(yī)務(wù)社會(huì)工作者本著“以患者為中心”的理念,及時(shí)向患者提供相關(guān)的醫(yī)療信息,通過(guò)相應(yīng)的社會(huì)工作手法和技巧的運(yùn)用,對(duì)患者進(jìn)行心理照顧和支持,從而起到平衡醫(yī)患信息,促進(jìn)醫(yī)患溝通,建立醫(yī)患之間的和諧關(guān)系。本文采用文獻(xiàn)研究法、訪談法、個(gè)案研究法等方法,首先以社會(huì)工作視角下對(duì)醫(yī)患關(guān)系進(jìn)行分析,發(fā)現(xiàn)了目前醫(yī)患關(guān)系的問(wèn)題:患者對(duì)醫(yī)生期望過(guò)高、目前醫(yī)患糾紛解決不善、患者和醫(yī)生溝通不暢,其中醫(yī)患溝通是最為主要的。接下來(lái)闡述了社會(huì)工作從三個(gè)方面介入醫(yī)患關(guān)系,并分析了個(gè)案方法處理運(yùn)用醫(yī)患溝通的必要性。在此基礎(chǔ)上引出具體實(shí)務(wù)分析,筆者以在成都市H醫(yī)院中遇到的真實(shí)案例為背景,嘗試個(gè)案工作方法處理運(yùn)用醫(yī)患溝通,根據(jù)分析,選定服務(wù)對(duì)象,進(jìn)行詳細(xì)評(píng)估,設(shè)計(jì)服務(wù)方案,呈現(xiàn)完整的個(gè)案服務(wù)過(guò)程,并以此總結(jié)經(jīng)驗(yàn),進(jìn)行反思。本文不同于以往從宏觀角度對(duì)醫(yī)患關(guān)系的研究,另辟蹊徑,從微觀層面著重對(duì)醫(yī)患溝通的介入,具有一定創(chuàng)新之處,有助于促進(jìn)醫(yī)務(wù)社會(huì)工作的本土化發(fā)展,并且對(duì)構(gòu)建和諧的醫(yī)患關(guān)系具有重要意義。
[Abstract]:The White Paper on the status of Medical Practitioners in China published in 2015 shows that the number of injuries and medical incidents is gradually increasing. This indicates that the doctor-patient relationship is deteriorating. I am fortunate to be a volunteer in medical social work. Taking this opportunity to learn about the current state of tension between doctors and patients. As people become increasingly concerned with medical services, people are beginning to realize that there is a fundamental need to prevent and treat diseases, We must consider and pay attention to the social and psychological factors of patients. Social work has other unparalleled advantages in this respect. In intervening in doctor-patient relations, medical social workers are guided by the concept of "patient-centered". Timely provide patients with relevant medical information, through the use of the corresponding social work techniques and skills, psychological care and support to patients, so as to balance the doctor-patient information, promote doctor-patient communication, In this paper, literature research, interview, case study and other methods are used to analyze the doctor-patient relationship from the perspective of social work. Problems in the current doctor-patient relationship have been identified: patients' expectations of doctors are too high, current doctor-patient disputes are poorly resolved, patients and doctors have trouble communicating, Among them, doctor-patient communication is the most important one. Then it expounds the three aspects of social work involved in the doctor-patient relationship, and analyzes the necessity of using the case method to deal with the doctor-patient communication. On the basis of this, it leads to the concrete practical analysis. In the background of the real cases in Chengdu H Hospital, the author tries to use the case work method to deal with the communication between doctors and patients, according to the analysis, to select the service object, to carry on the detailed evaluation, to design the service plan, to present the complete case service process. This paper is different from the previous studies on the relationship between doctors and patients from the macro perspective, and it is innovative to focus on the intervention of doctor-patient communication from the micro level. It is helpful to promote the localization of medical social work and to build harmonious doctor-patient relationship.
