【摘要】：基本醫(yī)療保險(xiǎn)是一項(xiàng)關(guān)系百姓切身利益的重要民生工程。隨著人們生活水平的不斷提高、基本醫(yī)療保險(xiǎn)制度的逐步健全,社會(huì)民眾對(duì)社會(huì)公平、醫(yī)療質(zhì)量和服務(wù)水平都提出了更高的希望與要求,然而我國(guó)城鄉(xiāng)二元社會(huì)經(jīng)濟(jì)結(jié)構(gòu)的存在,實(shí)行城鄉(xiāng)分割的基本醫(yī)療保險(xiǎn)管理理念與運(yùn)行模式,呈現(xiàn)出的城鄉(xiāng)二元特征難以彰顯社會(huì)保障的公正、公平,也阻礙著我國(guó)經(jīng)濟(jì)社會(huì)的協(xié)調(diào)、全面、可持續(xù)發(fā)展。因此,推進(jìn)城鄉(xiāng)公共服務(wù)均等化、平穩(wěn)推進(jìn)城市化進(jìn)程,將現(xiàn)有的碎片化、二元化基本醫(yī)療保險(xiǎn)制度進(jìn)行系統(tǒng)性、制度化整合,迅速有效地實(shí)現(xiàn)城鄉(xiāng)一體化的基本醫(yī)療保險(xiǎn)制度整合已經(jīng)成為擺在我國(guó)醫(yī)藥衛(wèi)生體制改革中一道現(xiàn)實(shí)迫切的課題。本文立足于湖州市區(qū)新型農(nóng)村合作醫(yī)療和城鎮(zhèn)居民基本醫(yī)療保險(xiǎn)制度基本框架,在充分考慮湖州市區(qū)現(xiàn)有基本醫(yī)療保險(xiǎn)制度基礎(chǔ)以及經(jīng)濟(jì)狀況的情況下,通過(guò)理論分析、社會(huì)調(diào)查和數(shù)據(jù)測(cè)算等方式,研究湖州市區(qū)城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)管理體制及相應(yīng)的保障能力。以歷年來(lái)湖州市區(qū)新型農(nóng)村合作醫(yī)療和城鎮(zhèn)居民基本醫(yī)療保險(xiǎn)基金收支數(shù)據(jù)為支撐,系統(tǒng)地調(diào)查城鄉(xiāng)居民對(duì)城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)的意見和要求,發(fā)現(xiàn)城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)整合過(guò)程中存在的難點(diǎn)與問(wèn)題,進(jìn)而結(jié)合個(gè)人工作經(jīng)歷提出相應(yīng)的對(duì)策與整合總體思路,形成湖州市區(qū)城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)制度整合的基本內(nèi)容。最后,設(shè)計(jì)出實(shí)用性較強(qiáng)的制度整合路徑設(shè)計(jì)模型。
[Abstract]:Basic medical insurance is an important project related to the vital interests of the people. With the continuous improvement of people's living standards, the gradual improvement of the basic medical insurance system, the social public is fair to the society. Both medical quality and service level have put forward higher hopes and requirements. However, with the existence of urban-rural dual social and economic structure, the basic medical insurance management concept and operation mode of urban-rural division are implemented. The dualistic characteristics of urban and rural areas are difficult to show the fairness and fairness of social security, and also hinder the coordination, comprehensive and sustainable development of our country's economy and society. Therefore, we should promote the equalization of urban and rural public services and promote the process of urbanization smoothly. To systematize and institutionalize the existing fragmented and dualized basic medical insurance system, The rapid and effective realization of the integration of the basic medical insurance system between urban and rural areas has become a realistic and urgent issue in the reform of the medical and health system of our country. This paper is based on the new rural cooperative medical system and towns in Huzhou urban area. The basic framework of the basic medical insurance system for residents, Having fully considered the basis of the existing basic medical insurance system and the economic situation in the urban areas of Huzhou, through theoretical analysis, social investigation and data calculation, This paper studies the basic medical insurance management system and the corresponding security ability of urban and rural residents in Huzhou. It is supported by the income and expenditure data of the new rural cooperative medical insurance and the basic medical insurance fund of urban residents in Huzhou over the years. To investigate systematically the opinions and requirements of urban and rural residents on basic medical insurance for urban and rural residents, and to find out the difficulties and problems existing in the process of integrating basic medical insurance for urban and rural residents. Then the author puts forward the corresponding countermeasures and the overall idea of integration, forms the basic content of the basic medical insurance system integration of urban and rural residents in Huzhou urban area. Finally, designs a practical system integration path design model.
