【摘要】：目的觀察P 物質(zhì)在過(guò)敏性猝死人體肺臟、咽喉和胃腸組織的表達(dá),應(yīng)用組合染色方法顯示肥大細(xì)胞脫顆粒現(xiàn)象,并探討其在法醫(yī)學(xué)鑒定中的作用和意義,試圖為過(guò)敏性猝死的法醫(yī)學(xué)鑒定提供可靠的形態(tài)學(xué)依據(jù)。 方法(1)應(yīng)用免疫組化方法對(duì)15 例過(guò)敏性猝死者肺臟、咽喉和胃腸組織的P 物質(zhì)表達(dá)進(jìn)行研究,同時(shí)以20 例冠心病猝死者作為對(duì)照組;應(yīng)用BI-2000 計(jì)算機(jī)圖像分析系統(tǒng)對(duì)P 物質(zhì)免疫組化染色結(jié)果進(jìn)行分析,計(jì)算陽(yáng)性指數(shù)(PI)。(2)應(yīng)用俾士麥棕-碘綠-馬汀黃(BB-IG-MY)組合染色法顯示肥大細(xì)胞脫顆�，F(xiàn)象。 結(jié)果(1)與對(duì)照組相比,實(shí)驗(yàn)組咽喉和胃腸組織P 物質(zhì)表達(dá)明顯增加。統(tǒng)計(jì)學(xué)t 檢驗(yàn)結(jié)果為:咽喉粘膜組t=6.942,咽喉腺體組t=4.601,胃腸組t=5.025,全部p 值小于0.001,均有統(tǒng)計(jì)學(xué)意義。(2)實(shí)驗(yàn)組和對(duì)照組肺組織細(xì)支氣管壁周圍未見(jiàn)P 物質(zhì)陽(yáng)性染色。(3)組合染色法結(jié)果表明,肥大細(xì)胞胞質(zhì)顆粒呈深棕色,細(xì)胞核呈綠色,紅細(xì)胞呈黃色;對(duì)照組肥大細(xì)胞未見(jiàn)脫顆粒現(xiàn)象,而實(shí)驗(yàn)組肥大細(xì)胞脫顆�，F(xiàn)象明顯。 結(jié)論過(guò)敏性猝死時(shí),咽喉、胃腸組織的P 物質(zhì)表達(dá)明顯增加,BB-IG-MY 組合染色法可清晰顯示過(guò)敏時(shí)肥大細(xì)胞脫顆粒現(xiàn)象。這些指標(biāo)可作為過(guò)敏性猝死的法醫(yī)學(xué)鑒定的形態(tài)學(xué)依據(jù)和參考指標(biāo)。
[Abstract]:Objective to observe the expression of substance P in the lung, throat and gastrointestinal tissues of patients with sudden allergic death, and to demonstrate the degranulation of mast cells by combined staining, and to explore the role and significance of substance P in forensic identification. This paper attempts to provide a reliable morphological basis for forensic identification of sudden allergic death. Methods (1) Immunohistochemical method was used to study the expression of substance P in lung, throat and gastrointestinal tissues of 15 patients with sudden allergic death and 20 patients with sudden death of coronary heart disease as control group. The immunohistochemical staining results of substance P were analyzed by BI-2000 computer image analysis system. The positive index (Pi). (2) was calculated and the mast cell degranulation was demonstrated by the combination staining method of Pygmy brown iodide green marting yellow (BB-IG-MY). Results (1) compared with the control group, the expression of substance P in the throat and gastrointestinal tissue of the experimental group was significantly increased. The results of t test were as follows: t of throat mucosa group was 6.942, of throat gland group was 4.601, of gastrointestinal group was 5.025, all p values were less than 0.001. (2) there was no positive staining of substance P around bronchiolus wall in experimental group and control group. (3) combination group. The results of staining showed that, The cytoplasmic granules of mast cells were dark brown, the nuclei were green and the red blood cells were yellow. In the control group, the degranulation of mast cells was not seen, but the degranulation of mast cells in the experimental group was obvious. Conclusion during sudden anaphylactic death, the expression of substance P in larynx and gastrointestinal tissues increases significantly by BB-IG-MY combined staining method, which can clearly show the degranulation of mast cells in allergy. These indexes can be used as morphological basis and reference index for forensic identification of sudden allergic death.
