【摘要】: 目的:研究摩托車交通事故傷亡的流行病學(xué)特點和損傷特征;探討摩托車交通事故受傷人員死亡和傷殘的影響歷素,分析受傷人員的傷情鑒定、傷殘評定和后期醫(yī)療費用特點及相關(guān)因素,為預(yù)防、控制道路交通事故、創(chuàng)傷急救和法醫(yī)學(xué)應(yīng)用提供依據(jù)。方法:采用醫(yī)學(xué)統(tǒng)計學(xué)方法和法醫(yī)學(xué)技術(shù)對2004年和2005年昆明地區(qū)摩托車道路交通事故365例傷亡的法醫(yī)學(xué)鑒定資料進行系統(tǒng)性分析研究。結(jié)果:本組365例摩托車交通事故案例中男女比例為4.14:1,,年齡呈偏態(tài)分布,21~40歲占傷亡人數(shù)的62.7%。職業(yè)分布以農(nóng)民最多。事故發(fā)生的時間分布19~0時最高,占36.4%。貨車在肇事車輛中的構(gòu)成比最大,肇事方式以機動車與摩托車相撞為主,事故方向多為相向所致。傷亡人員中,摩托車駕駛員最多。死亡原因以顱腦損傷多見;損傷多由碰撞和摔跌形成,損傷部位呈離心性分布。損傷程度以輕傷為主,頭面部的損傷率在重傷中高于輕傷和輕微傷,重傷在老年人中的構(gòu)成比高于中青年人和少年兒童。傷殘評定結(jié)果中,Ⅸ級-Ⅹ級占87.6%。后期費用主要集中在2千元~4千元,其次為6千~8千元。住院時間為15-30天所占比例最高;傷殘評定時間主要分布在傷后3~6個月。摩托車駕駛員年齡分布呈偏態(tài),眾數(shù)25歲,21歲-40歲占69.8%。摩托車駕駛員的死亡率和傷殘率最高。胸部、腹盆部、會陰、手背和大腿內(nèi)側(cè)的損傷率高于摩托車乘員。損傷程度以輕傷多見,傷殘程度以X級居多。摩托車交通事故中死亡的相關(guān)因素依次為顱腦損傷、碾壓傷和胸廓骨折;傷殘的相關(guān)因素為腹盆部損傷、顱腦損傷、下肢骨折和上肢骨折。結(jié)論:摩托車交通事故受傷害人群以中青年為主,致死致殘率高,事故時間以19~0時最多,1~6時事故的死率最高。摩托車駕乘人員是最主要的受害者,機動車駕駛員最少。肇事方式以機動車與摩托車為主。不同交通方式人員的損傷分布總體有差異。損傷程度以輕傷為主,隨著年齡的增大,重傷的發(fā)生率也逐步增大。傷殘程度以輕度傷殘為主,老年人的傷殘率高于中青年人和少年兒童。胸部、腹盆部、會陰部和上、下肢的損傷是摩托車駕駛員較為特征性的損傷。摩托車交通事故中死亡與顱腦損傷、碾壓傷和胸廓骨折相關(guān)性較大;傷殘與腹盆部損傷、顱腦損傷、下肢骨折和上肢骨折相關(guān)性較大。
[Abstract]:Objective: to study the epidemiological characteristics and injury characteristics of motorcycle traffic accident casualties, to explore the effects of death and disability on motorcycle traffic accident victims, and to analyze the identification of injury situation of the injured persons. The characteristics and related factors of disability assessment and later medical expenses, in order to prevent and control road traffic accidents, To provide evidence for trauma first aid and forensic application. Methods: the forensic identification data of 365 motorcycle traffic accidents in Kunming on 2004 and 2005 were systematically classified by medical statistics and forensic techniques. Results: the ratio of male to female was 4.14: 1 in 365 cases of motorcycle traffic accident. The age distribution was skewed. The age distribution was 21 40 years old, which accounted for 62.7% of the casualties. The occupation distribution was the most among the peasants. The time distribution of the accident was the highest from 19 to 0:00. The composition ratio of serious injury in the elderly was higher than that in the middle and young people and children. In the disability assessment results, grade IX-X accounted for 87.66.The expenses in the later period were mainly 2,000 yuan ~ 4,000 yuan, followed by 6,000 ~ 8,000 yuan. The proportion of hospital stay was the highest in 15-30 days. The age distribution of motorcyclists was skewed, and 69.8% of them were aged from 21 to 40 years old. The mortality and disability rate of motorcycle drivers were the highest, chest, abdomen and pelvis, perineum, perineum and perineum. The injury rate of the inner side of the hand and thigh was higher than that of the motorcycle crew. The degree of injury was mostly light injury, and the degree of disability was mostly grade X. the related factors of death in motorcycle traffic accident were craniocerebral injury, crush injury and thoracic fracture. The related factors of disability were abdominal and pelvic injury, craniocerebral injury, lower limb fracture and upper limb fracture. The time of the accident was at most 1 ~ 6:00 from 19 to 0:00. The motorcyclists were the main victims. The driver of motor vehicle is the least. The main way of accident is motor vehicle and motorcycle. There are differences in the distribution of injuries among different traffic mode personnel. The degree of injury is mainly light injury, and with the increase of age, The incidence of serious injury also gradually increased. The degree of disability was mainly mild disability. The disability rate of the elderly was higher than that of the young and middle-aged people and children, the chest, abdomen and pelvis, perineum and upper, The injuries of lower limbs are characteristic injuries of motorcyclists. There is a great correlation between death and craniocerebral injury, crush injury and thoracic fracture in motorcycle traffic accident, injury of abdomen and pelvis, craniocerebral injury, injury of abdomen and pelvis, craniocerebral injury, injury of abdomen and pelvis. Lower extremity fracture and upper limb fracture are closely related.
