人體組織的過敏性休克實驗研究

【摘要】:肥大細胞(Mast Cell, MC)、嗜酸性粒細胞與變態(tài)反應性疾病有著密切的關系,在其發(fā)病過程中發(fā)揮重要的作用。過敏性休克是臨床常見的急癥之一,由于缺乏確切的實驗室診斷標準,是導致醫(yī)療糾紛的一個重要原因,在非法行醫(yī)案件中也時有發(fā)生。因此,研究過敏性休克發(fā)生的機制,確立可靠、準確的診斷指標,找出簡單易行的診斷方法,在法醫(yī)病理診斷中尤為重要。本實驗應用類胰蛋白酶(Tryptase)、白細胞介素2受體(IL-2R)、嗜酸性粒細胞趨化因子(Eotaxin)、腫瘤壞死因子α(TNF-α)免疫組化染色及檢測Eotaxin基因陽性表達的原位雜交方法觀察了30例過敏性休克致死和8例非過敏性休克致死病例的各臟器,結果如下: 通過以上研究,得到如下結論: 1.過敏性休克致死尸檢病例中肺組織、腸、喉組織肥大細胞(MC)數(shù)目增多。 2.類胰蛋白酶可以作為MC特異性標記檢測組織中MC的數(shù)量。 3.尸檢病例腸及肺等組織中大量嗜酸性粒細胞的存在提示過敏性休克死亡。 4.TNF-α、IL-2Rα參與過敏性休克的病理過程。 5.過敏性休克時,肺、腎、腸、喉Eotaxin基因陽性表達染色陽性。Eotaxin免疫組化染色陽性。 本實驗應用免疫組化染色和原位雜交的方法,通過研究過敏性休克中MC的形態(tài)變化,證實在過敏性休克過程中類胰蛋白酶、TNF-α、IL-2Rα和Eotaxin發(fā)揮的作用,為其在法醫(yī)病理實踐中的應用提供了切實的理論依據(jù)。
[Abstract]:Mast cell, MCN, eosinophil are closely related to allergic diseases and play an important role in the pathogenesis. Anaphylactic shock is one of the common clinical emergencies, due to the lack of accurate laboratory diagnostic criteria. Is an important cause of medical disputes, and also occurs in illegal medical cases. Therefore, to study the mechanism of anaphylactic shock, to establish reliable and accurate diagnostic indicators, to find out a simple and easy diagnostic method, In this study, we used trypsin like trypsin, interleukin-2 receptor IL-2R, eosinophil chemokine eotaxin, tumor necrosis factor 偽 -TNF- 偽 to detect the expression of Eotaxin gene in situ. The viscera of 30 cases of anaphylactic shock and 8 cases of non-anaphylactic shock were observed by hybridization. The results are as follows:. Through the above research, the following conclusions are drawn:. 1. The number of mast cells in lung, intestine and larynx was increased in the autopsy patients with anaphylactic shock. 2. Trypsin can be used as a specific marker to detect the number of MC in tissues. 3. The presence of a large number of eosinophils in intestinal and lung tissues suggests anaphylactic shock death. 4. TNF- 偽 and IL-2R 偽 participate in the pathological process of anaphylactic shock. 5. In anaphylactic shock, Eotaxin gene expression was positive in lung, kidney, intestine and larynx. In this study, immunohistochemical staining and in situ hybridization were used to study the morphologic changes of MC in anaphylactic shock, and to confirm the role of TNF- 偽 IL-2R 偽 and Eotaxin in the process of anaphylactic shock. It provides a practical theoretical basis for its application in forensic pathology practice.
