初次膝關(guān)節(jié)表面置換術(shù)后切口皮溫測(cè)定和冰敷的臨床效果

【摘要】:目的探討膝關(guān)節(jié)置換(TKA)術(shù)后正常情況下切口局部早期的皮溫變化規(guī)律及冰敷治療的初步臨床療效。方法選擇接受單側(cè)初次膝關(guān)節(jié)表面置換術(shù)的患者共281例。隨機(jī)分為對(duì)照組140例,觀察組141例,對(duì)照組予以常規(guī)護(hù)理和皮溫測(cè)定,觀察組在此基礎(chǔ)上術(shù)后予以持續(xù)冰敷治療12周。對(duì)所有患者進(jìn)行術(shù)后前5 d,第14天,第6周和第12周雙膝關(guān)節(jié)皮溫的監(jiān)測(cè),觀察兩組患者皮溫、疼痛評(píng)分及膝關(guān)節(jié)功能恢復(fù)情況。結(jié)果獲得隨訪281例,其中女性172例,男性109例,平均年齡(74.2±6.39)歲,隨訪時(shí)間第1天~第12周。對(duì)照組平均皮溫差:術(shù)后第3天為(3.2±0.4)℃,第5天為(3.0±0.3)℃,第14天為(2.7±0.5)℃,第6周為(2.6±0.2)℃,第12周為(2.5±0.2)℃。觀察組平均皮溫差:術(shù)后第3天為(1.2±0.4)℃,第5天為(1.1±0.3)℃,第14天為(1.2±0.5)℃,第6周為(1.0±0.2)℃,第12周為(0.9±0.2)℃。觀察組術(shù)后各個(gè)監(jiān)測(cè)時(shí)間點(diǎn)皮溫顯著低于對(duì)照組(P0.05)。兩組患者術(shù)后第6周、第12周的VAS評(píng)分均較術(shù)前明顯降低(P0.05),且觀察組患者同期VAS評(píng)分低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 TKA術(shù)后切口周圍皮溫是臨床評(píng)估膝關(guān)節(jié)恢復(fù)情況的參考指標(biāo)。膝關(guān)節(jié)置換術(shù)患者予以早期冰敷治療,可有效減輕疼痛,減低切開(kāi)局部皮溫,促進(jìn)膝關(guān)節(jié)功能盡快恢復(fù)。

On this basis, the observation group was treated with continuous ice compress for 12 weeks. The skin temperature of double knee joint was monitored 5 days before operation, 14 days, 6 weeks and 12 weeks after operation.
