2011-2015年某院分離3種常見腸桿菌分布及耐藥性分析

【摘要】:目的研究2011-2015年某院常見腸桿菌的分離及耐藥情況,為臨床合理用藥提供參考。方法收集該院5年分離的常見腸桿菌,Vitek 2 Compact全自動微生物分析系統(tǒng)結(jié)合基質(zhì)輔助激光解吸電離飛行時間質(zhì)譜儀(MALDI-TOF)完成細(xì)菌鑒定,K-B法進(jìn)行藥敏實驗,WHONET 5.6軟件數(shù)據(jù)分析。結(jié)果 5年分離常見腸桿菌6 908株,總分離率為44.7%。標(biāo)本主要為痰液,占39.1%,其次為尿液和傷口拭子,分別占15.3%和7.9%。大腸埃希菌對青霉素類、頭孢菌素類或喹諾酮類等藥物的耐藥率大于50.0%,對酶抑制劑復(fù)合制劑和碳青霉烯類藥物的耐藥率分別低于15.0%和2.0%。產(chǎn)超廣譜β-內(nèi)酰胺酶(Extended-Spectrum Beta-Lactamase,ESBLs)的肺炎克雷伯菌維持在28.4%~34.8%,肺炎克雷伯菌對各類藥物的耐藥率均小于50.0%。5年間,陰溝腸桿菌的ESBLs陽性率逐年升高,2015年達(dá)100.0%,陰溝腸桿菌對青霉素類、磺胺類、頭孢菌素類、酶抑制劑復(fù)合制劑等藥物的耐藥率較高;對亞胺培南和美羅培南最為敏感,耐藥率在10.0%以下。結(jié)論該院近5年分離的常見腸桿菌呈現(xiàn)不同程度耐藥,臨床治療應(yīng)根據(jù)藥敏結(jié)果選擇合理的抗菌藥物。
[Abstract]:Objective to study the isolation and drug resistance of common enterobacillus in a hospital from 2011 to 2015, and to provide reference for clinical rational drug use. Methods Vitek 2 Compact automatic microbial analysis system combined with matrix assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF) was used to identify common Enterobacteriaceae isolated in our hospital for 5 years. Drug sensitivity test and WHONET 5.6 software data analysis were carried out by K / B method. Results 6908 strains of Enterobacteriaceae were isolated in 5 years, and the total isolation rate was 44.7%. The main specimens were sputum (39.1%), followed by urine and wound swabs (15.3% and 7.9%, respectively). The resistance rate of Escherichia coli to penicillin, cephalosporins or quinolones was more than 50.0%, and to enzyme inhibitor and carbapenem was less than 15.0% and 2.0%, respectively. Klebsiella pneumoniae producing extended-spectrum 尾-lactamase (Extended-Spectrum Beta-Lactamase,ESBLs) remained at 28.4% 鈮,
