海南省219株耐藥結(jié)核分枝桿菌藥敏試驗(yàn)結(jié)果分析

【摘要】:目的分析海南省結(jié)核病醫(yī)院502株結(jié)核菌株的耐藥類型及分布情況,為制定耐藥結(jié)核病治療方案提供科學(xué)依據(jù)。方法收集2014年1月至2016年12月期間收治的經(jīng)痰分離培養(yǎng)并鑒定為結(jié)核分枝桿菌復(fù)合群的共502株菌株,進(jìn)行8種抗結(jié)核藥物(異煙腆、利福平、乙胺丁醇、鏈霉素、卡那霉素、卷曲霉素、氧氟沙星以及丙硫異煙胺)的藥物敏感性試驗(yàn),對(duì)其結(jié)果進(jìn)行分析。結(jié)果總耐藥率為43.6%(219/502),2015、2016年總耐藥率明顯高于2014年(=10.136,P=0.001;=8.56,P=0.003);總耐多藥率為25.5%(128/502),近3年耐多藥疫情無(wú)明顯變化(=3.297,P0.05);抗結(jié)核藥物耐藥率由高到低依次為利福平異煙肼鏈霉素氧氟沙星乙胺丁醇卡那霉素卷曲霉素丙硫異煙胺;耐藥類型以耐多藥和利福平耐藥為主。結(jié)論海南省耐藥結(jié)核病尤其是耐多藥結(jié)核病形勢(shì)不容樂(lè)觀;乙胺丁醇、卡那霉素、卷曲霉素、丙硫異煙胺在海南地區(qū)仍有很大的臨床抗結(jié)核價(jià)值,臨床上應(yīng)予以關(guān)注;應(yīng)繼續(xù)加強(qiáng)耐藥結(jié)核病發(fā)現(xiàn)力度和防控策略,遏制耐藥結(jié)核病流行。
