【摘要】：總結(jié)了6例重型再生障礙性貧血合并彌漫性血管內(nèi)凝血(disseminated intravascular coagulation,DIC)患者的護(hù)理經(jīng)驗(yàn)。通過積極預(yù)防口腔、呼吸道、皮膚、肛周感染,可以降低敗血癥、重癥肺炎發(fā)生;監(jiān)測(cè)出凝血指標(biāo),判斷DIC疾病時(shí)相、出血表現(xiàn),減少皮膚穿刺,局部用藥、冷敷、壓迫,可以減少皮膚、鼻腔、口腔出血;嚴(yán)格控制肝素滴速、監(jiān)測(cè)出凝血指標(biāo)、隨時(shí)向醫(yī)生匯報(bào)、掌握處理肝素過量的處理方法,可以增加肝素用藥的安全性,提高早期DIC肝素治療的臨床應(yīng)用率;心理指導(dǎo)可以降低患者用力過度、意外摔倒導(dǎo)致腦出血發(fā)生。經(jīng)過治療和精心護(hù)理,6例患者好轉(zhuǎn)出院5例,死亡1例。
[Abstract]:The nursing experience of 6 patients with severe aplastic anemia complicated with disseminated intravascular coagulation (disseminated intravascular coagulation,DIC was summarized. By actively preventing oral, respiratory, skin and perianal infections, septicemia and severe pneumonia can be reduced. Monitoring coagulation indexes, judging the phase of DIC disease, bleeding manifestations, reducing skin puncture, local drug use, cold compress, compression, can reduce skin, nasal cavity, oral bleeding; Strictly controlling the speed of heparin drip, monitoring the coagulation index, reporting to the doctor at any time, mastering the treatment method of heparin excess can increase the safety of heparin treatment and improve the clinical application rate of early DIC heparin treatment. Psychological guidance can reduce the excessive force of patients, accidental fall leading to cerebral hemorrhage. After treatment and careful nursing, 5 cases were discharged from hospital and 1 case died.
