【摘要】：目的探討穿刺側(cè)上肢不同外展角度對PICC頸內(nèi)靜脈異位的影響。方法選取我院2015年8月—2017年1月行PICC置管單側(cè)乳腺癌患者210例作為研究對象,隨機分為3組,每組70例,3組穿刺側(cè)上肢外展角度分別設為45°、90°、160°,置管靜脈均選擇健側(cè)貴要靜脈,采用超聲引導下改良塞丁格技術肘上置管,導管送入預定長度后,進行B超檢查,如果在頸內(nèi)靜脈發(fā)現(xiàn)導管末端顯影,即判定為頸內(nèi)靜脈異位。結(jié)果 45°組發(fā)生頸內(nèi)靜脈異位率為7.14%(5例),90°組為8.57%(6例),160°組未發(fā)生異位;3組比較,差異無統(tǒng)計學意義(χ2=5.95,P0.05,);兩兩比較,160°組與45°組、160°組和90°組,差異有統(tǒng)計學意義(P0.05);45°組和90°組,差異無統(tǒng)計學意義(P0.05)。結(jié)論 45°、90°和160°均可作為PICC的置管角度,可以不僅僅局限于90°,可根據(jù)患者具體情況選擇置管角度,制訂個性化的置管方案。
[Abstract]:Objective to investigate the effect of different abduction angles of puncture side upper limb on ectopic internal jugular vein of PICC. Methods from August 2015 to January 2017, 210 patients with unilateral breast cancer treated with PICC catheterization were randomly divided into three groups, 70 patients in each group. The abduction angles of the upper limbs in the three groups were 45 擄, 90 擄and 160 擄, respectively. All the catheterization veins selected the contralateral vein, and the modified Sedinger technique was used to place the tube on the elbow under the guidance of ultrasound. After the catheter was sent into the predetermined length, B-ultrasound examination was carried out. If the end of the catheter was found in the internal jugular vein, it was determined to be ectopic of the internal jugular vein. Results the ectopic rate of internal jugular vein was 7.14% (5 cases) in 45 擄group, 8.57% (6 cases) in 90 擄group, and no heterosis occurred in 160 擄group, there was no significant difference among the three groups (蠂 2 鈮，
