【摘要】：目的對(duì)"社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量評(píng)價(jià)指標(biāo)體系"進(jìn)行實(shí)證研究,以了解社區(qū)產(chǎn)后家庭訪視護(hù)理服務(wù)開(kāi)展情況及存在問(wèn)題,為今后完善社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量評(píng)價(jià)提供實(shí)用性評(píng)估工具和理論依據(jù)。方法于2016年10—12月,以3∶1的比例在銀川市3個(gè)市轄區(qū)中隨機(jī)抽取社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)18家。根據(jù)"社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量評(píng)價(jià)指標(biāo)體系",結(jié)合社區(qū)產(chǎn)后家庭訪視護(hù)理工作現(xiàn)狀,設(shè)計(jì)相應(yīng)評(píng)價(jià)量表和調(diào)查問(wèn)卷,由科研小組成員進(jìn)行實(shí)地調(diào)查和考評(píng)。其中,過(guò)程質(zhì)量評(píng)價(jià)采用《社區(qū)產(chǎn)后家庭訪視護(hù)理過(guò)程質(zhì)量現(xiàn)場(chǎng)考察表》,結(jié)構(gòu)質(zhì)量、結(jié)果質(zhì)量評(píng)價(jià)采用《社區(qū)產(chǎn)后家庭訪視護(hù)理服務(wù)定性及定量評(píng)價(jià)指標(biāo)調(diào)查表》《社區(qū)產(chǎn)后家庭訪視護(hù)理工作人員滿意度調(diào)查問(wèn)卷》《產(chǎn)婦滿意度及相關(guān)健康知識(shí)知曉程度調(diào)查表》。調(diào)查數(shù)據(jù)經(jīng)標(biāo)準(zhǔn)化轉(zhuǎn)換后得到各指標(biāo)的標(biāo)準(zhǔn)得分,然后采用加權(quán)累加綜合評(píng)分法計(jì)算各社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)的社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量得分。結(jié)果 18家社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)中,社區(qū)產(chǎn)后訪視人員中的醫(yī)生數(shù)均為0、社區(qū)護(hù)士數(shù)均為1,訪視人員年崗位培訓(xùn)率、年度理論和技能考核合格率均為0;對(duì)18家社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)社區(qū)產(chǎn)后訪視前準(zhǔn)備、社區(qū)產(chǎn)后訪視中新生兒一般情況評(píng)估、社區(qū)產(chǎn)后訪視中產(chǎn)婦體格檢查的實(shí)施情況評(píng)價(jià)為非常好/好的構(gòu)成比分別為86.10%、85.54%、86.07%,對(duì)訪視中產(chǎn)婦一般情況評(píng)估的實(shí)施情況評(píng)價(jià)為非常好/好的構(gòu)成比為22.25%;訪視人員對(duì)現(xiàn)有產(chǎn)后訪視條件的平均滿意度為89.86%,產(chǎn)婦對(duì)訪視人員訪視時(shí)間/次數(shù)/內(nèi)容的平均滿意度為73.20%。18家社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)的社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量總得分為65.45~84.13分,平均分為70.55分,分值跨度為18.68分。結(jié)論 "社區(qū)產(chǎn)后家庭訪視護(hù)理質(zhì)量評(píng)價(jià)指標(biāo)體系"覆蓋內(nèi)容較為科學(xué)、全面,具有一定的操作適宜性和實(shí)用價(jià)值;社區(qū)產(chǎn)后家庭訪視工作還存在人員崗位培訓(xùn)不到位、�？茩z查欠缺、產(chǎn)婦滿意度較低等問(wèn)題。
[Abstract]:Objective to make an empirical study on the evaluation index system of community postpartum home visit nursing quality in order to understand the situation and problems of community postpartum home visit nursing service. It provides a practical assessment tool and theoretical basis for improving the quality evaluation of post-natal family visit nursing in the future. Methods from October to December 2016, 18 community health service institutions were randomly selected from 3 districts of Yinchuan city at 3:1. According to the evaluation index system of community postpartum family visit nursing quality, combined with the present situation of community postpartum family visit nursing work, the corresponding evaluation scale and questionnaire were designed, and the field investigation and evaluation were carried out by the members of the scientific research group. Among them, the evaluation of process quality was based on the "Field investigation Table of Community Post-natal Family visit Nursing process", and the structural quality. Results the questionnaire of qualitative and quantitative evaluation index of community postpartum family visit nursing service was used to evaluate the quality of nursing. < questionnaire of satisfaction of community postpartum family visiting nursing staff > < questionnaire of knowledge of maternal satisfaction and related health knowledge]. The standard scores of each index were obtained after the data were standardized converted, and then the score of community postnatal home visit nursing quality was calculated by weighted cumulative comprehensive score method. Results among the 18 community health service institutions, the number of doctors and community nurses were 0 and 1 respectively. The annual post training rate and the qualified rate of annual theory and skill examination were all 0 among the 18 community health service institutions. In 18 community health service institutions, the preparation of postpartum visits, the assessment of the general situation of newborns in postnatal visits, and the implementation of physical examination of parturient women in postpartum visits in the community were evaluated to be very good / good composition ratios of 86.1010, respectively. 85.54 and 86.07, the evaluation of the implementation of the evaluation of the general situation of the pregnant women during the visit is very good / good composition ratio is 22.25; The average satisfaction of the interviewees with the existing conditions for post-natal visits was 89.86%. The average satisfaction of the time / frequency / content of the visits was 73.20.18 community health service institutions had a total score of 65.45 ~ 84.13 points, with an average score of 70.55 points. The span is 18.68. Conclusion the content of the evaluation index system of community postpartum home visit nursing quality is more scientific and comprehensive, and it has certain operational suitability and practical value. Community postpartum family visit work still exists the personnel post-training is not in place, the specialized examination is deficient, the maternal satisfaction degree is low and so on.
