【摘要】：目的探討機(jī)械輔助排痰結(jié)合體位引流與常規(guī)叩背吸痰對(duì)慢性阻塞性肺疾病急性發(fā)作(AECOPD)患者機(jī)械通氣脫機(jī)的影響。方法60例需要接受機(jī)械通氣的AECOPD患者用隨機(jī)數(shù)字法分為機(jī)械輔助排痰結(jié)合體位引流組(治療組,30例)和常規(guī)叩背吸痰組(對(duì)照組,30例),治療組使用機(jī)械排痰機(jī)結(jié)合體位引流治療,對(duì)照組采用人工叩背結(jié)合體位引流。分別記錄兩組年齡、性別、既往基礎(chǔ)疾病、急性生理與慢性健康狀況評(píng)分(APACHE)Ⅱ以及脫機(jī)前機(jī)械通氣時(shí)間等指標(biāo)。兩組患者除排痰方法不同外,其他處理均相同,統(tǒng)計(jì)兩組患者治療后的排痰量、氧分壓(Pa O_2)、二氧化碳分壓(Pa CO_2)、氣道阻力、總機(jī)械通氣時(shí)間、脫機(jī)時(shí)間、結(jié)果治療組患者氣道阻力降低、機(jī)械通氣時(shí)間均優(yōu)于對(duì)照組。治療組從分組到通過SBT的機(jī)械通氣時(shí)間和總機(jī)械通氣時(shí)間均短于對(duì)照組(P=0.001),氣道阻力降低優(yōu)于對(duì)照組(P=0.01);Pa O_2及Pa CO_2組間比較無統(tǒng)計(jì)學(xué)差異(P=0.134、P=0.073),脫機(jī)成功率、脫機(jī)48 h后接受無創(chuàng)通氣比率及再插管率組間無統(tǒng)計(jì)學(xué)差異(P=0.317、P=0.717、P=0.687),住院病死率組間比較無統(tǒng)計(jì)學(xué)差異(P=0.640)。結(jié)論機(jī)械輔助排痰結(jié)合體位引流有助于AECOPD患者機(jī)械通氣的脫機(jī)。
[Abstract]:Objective to investigate the effect of mechanical assisted sputum drainage combined with conventional sputum tapping on mechanical ventilation weaning in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).
