不同電壓的脈沖射頻對(duì)大鼠CCI模型痛閾值及神經(jīng)結(jié)構(gòu)的影響

【摘要】:目的:觀察不同電壓的脈沖射頻(PRF)對(duì)大鼠坐骨神經(jīng)慢性縮窄損傷(CCI)疼痛模型熱痛閾(TWL)、機(jī)械痛閾(MWT)及坐骨神經(jīng)結(jié)構(gòu)的影響。方法:72只成年雄性SD大鼠,隨機(jī)均分為4組:假射頻組、PRF-45V組、PRF-55V組、PRF-65V組,每組18只。CCI模型建立成功后,假射頻組僅放置射頻針120s而無(wú)脈沖干預(yù),其余三組分別行45V、55V與65V電壓的PRF干預(yù)(頻率2Hz、時(shí)間120s)。分別在制模前、模型成立(即制模后7 d)、PRF后1、4、7、10和14 d檢測(cè)各大鼠的TWL值和MWT值的變化。并于PRF后14 d取大鼠右側(cè)PRF干預(yù)處坐骨神經(jīng)干,電鏡下觀察其結(jié)構(gòu)變化。結(jié)果:模型成立后各組大鼠TWL值和MWT值均顯著低于制模前,其差異有統(tǒng)計(jì)學(xué)意義(P0.05);制模前和模型成立時(shí)各組大鼠TWL值和MWT值比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);與假射頻組比較,PRF-45V組、PRF-55V組和PRF-65V組在PRF干預(yù)后各時(shí)點(diǎn)TWL值和MWT值均較高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);與PRF-45V組比較,PRF-55V組和PRF-65V組PRF后14 d時(shí)TWL值和MWT值均較高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。電鏡下各組大鼠坐骨神經(jīng)均無(wú)脫髓鞘等嚴(yán)重?fù)p傷,PRF-65V組可見大量外形不規(guī)則的髓鞘出現(xiàn),板層結(jié)構(gòu)腫脹變粗,且程度高于PRF-55V組和PRF-45V組。結(jié)論:1.CCI大鼠經(jīng)PRF干預(yù)后早期痛閾明顯恢復(fù),且高電壓55V和65V的痛閾恢復(fù)程度高于低電壓45V,55V與65V兩者間痛閾改變差異不明顯。2.本實(shí)驗(yàn)所設(shè)立電壓的PRF對(duì)坐骨神經(jīng)僅造成輕微損傷,PRF-65V組有髓神經(jīng)纖維水腫程度高于PRF-55V組和PRF-45V組。
[Abstract]:Aim: to observe the effects of pulsed radiofrequency (PRF) at different voltages on the thermal pain threshold (TWL), mechanical pain threshold (MWT) and the structure of sciatic nerve in rats with (CCI) pain induced by chronic constriction injury of sciatic nerve. Methods: 72 adult male SD rats were randomly divided into 4 groups: pseudo-radio frequency group, PRF-45V group, PRF-55V group and PRF-65V group, 18 rats in each group. After the successful establishment of CCI model, the pseudo-radio frequency group was only placed with radio frequency needle for 120 s without pulse intervention. The other three groups were treated with PRF at 45 V, 55 V and 65 V, respectively (frequency 2 Hz, time 120 s). The changes of TWL and MWT values of each rat were detected at 10 and 14 days after modeling (7 d), PRF after modeling). At 14 days after PRF, the sciatic nerve trunk was taken from the right side of PRF of rats, and the structural changes were observed under electron microscope. Results: the values of TWL and MWT in each group were significantly lower than those before modeling (P0.05), but there was no significant difference in TWL and MWT between the groups before model making and when the model was established (P0.05). Compared with the pseudo-radiofrequency group, the TWL and MWT values of PRF-45V group, PRF-55V group and PRF-65V group were higher at each time point after PRF intervention, the difference was statistically significant (P0.05). Compared with PRF-45V group, the TWL value and MWT value of PRF-55V group and PRF-65V group were higher at 14 days after PRF, the difference was statistically significant (P0.05). There was no serious injury of sciatic nerve such as demyelination under electron microscope. A large number of irregular myelin sheath appeared in PRF-65V group, and the lamellar structure was swollen and thickened, and the degree was higher than that in PRF-55V group and PRF-45V group. Conclusion: the early pain threshold of 1.CCI rats recovered obviously after PRF intervention, and the recovery degree of pain threshold of high voltage 55V and 65V was higher than that of low voltage 45V 55V and 65V respectively. 2. The degree of edema of myelinated nerve fibers in PRF-65V group was higher than that in PRF-55V group and PRF-45V group.
