【摘要】：目的:探討血清中胃蛋白酶原(PG)、胃泌素17(G-17)和幽門螺桿菌抗體(anti-HP)檢測應(yīng)用于胃癌篩查的可行性,闡明其臨床價(jià)值。方法:選擇早期胃癌患者208例作為觀察組,同期體檢正常者208人作為空白對照組。利用酶聯(lián)免疫吸附試驗(yàn)和乳膠增強(qiáng)免疫比濁法檢測2組受試者血清中PGⅠ、PGⅡ和G-17水平;采用13 C尿素呼吸試驗(yàn)檢測2組受試者anti-HP陽性率,并對anti-HP陽性(anti-HP陽性組)和anti-HP陰性(anti-HP陰性組)受試者血清PGⅠ、PGⅡ水平和PGⅠ/PGⅡ比值進(jìn)行比較分析。結(jié)果:與空白對照組比較,觀察組患者血清PGⅠ/PGⅡ比值和PGⅠ水平明顯降低(P0.01),而PGⅡ、G-17水平和anti-HP陽性率明顯升高(P0.01);與anti-HP陰性組比較,anti-HP陽性組受試者血清PGⅠ/PGⅡ比值和PGⅠ水平明顯降低(P0.01),而PGⅡ和G-17水平明顯升高(P0.01)。結(jié)論:anti-HP陽性率結(jié)合PGⅠ/PGⅡ比值檢測對于胃癌篩查具有重要的臨床意義,提示上皮內(nèi)瘤病變組織中G-17和PGⅡ水平可以作為胃癌前病變的觀察指標(biāo)。
