【摘要】：目的探討三級(jí)綜合醫(yī)院實(shí)施五步法綜合干預(yù)措施對(duì)提高手衛(wèi)生依從性和降低醫(yī)院多藥耐藥菌分離率的有效性。方法 2013年8月-2014年8月為干預(yù)前組,2014年9月-2015年8月為干預(yù)后組,采用直接觀察法調(diào)查手衛(wèi)生依從率,在獲取了依從性的基線水平后,實(shí)施綜合干預(yù)措施,通過χ~2檢驗(yàn)進(jìn)行干預(yù)前后手衛(wèi)生依從性、多藥耐藥菌分離率的數(shù)據(jù)處理和分析。結(jié)果全院醫(yī)務(wù)人員手衛(wèi)生依從率干預(yù)前為54.0%,逐步提高至干預(yù)后的74.0%,差異有統(tǒng)計(jì)學(xué)意義(P0.001);MRSA臨床標(biāo)本分離率從干預(yù)前的54.6%,下降至干預(yù)后的35.9%,VRE的臨床標(biāo)本分離率從干預(yù)前的30.1%,下降至干預(yù)后的9.2%,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論實(shí)施五步法綜合干預(yù)措施能明顯提高手衛(wèi)生依從性,有效降低醫(yī)院多藥耐藥菌的分離率。
[Abstract]:Objective to explore the effectiveness of five steps comprehensive intervention in tertiary general hospitals to improve hand hygiene compliance and reduce the isolation rate of multidrug resistant bacteria in hospitals. Methods from August 2013 to August 2014 as the pre-intervention group and from September 2014 to August 2015 as the post-intervention group, the hand hygiene compliance rate was investigated by direct observation method. After obtaining the baseline level of compliance, comprehensive intervention was carried out. The data processing and analysis of hand hygiene compliance and multidrug resistant bacteria isolation rate before and after intervention were carried out by 蠂 ~ 2 test. Results the hand hygiene compliance rate of medical staff in the whole hospital was 54.0 before intervention, and gradually increased to 74.0 after intervention, the difference being statistically significant (P0.001). The isolation rate of MRSA clinical specimens decreased from 54.6 before intervention to 35.9VRE after intervention from 30.1 to 9.2 after intervention (P0.01). Conclusion five-step comprehensive intervention can significantly improve hand hygiene compliance and effectively reduce the isolation rate of multidrug resistant bacteria in hospitals.
