【摘要】：目的:在了解國內(nèi)外重癥�？谱o(hù)士培訓(xùn)現(xiàn)狀的基礎(chǔ)上,建立重慶市重癥醫(yī)學(xué)專科護(hù)士實(shí)踐培訓(xùn)體系,包括培訓(xùn)內(nèi)容、課程設(shè)置、培訓(xùn)效果等方面的評價(jià),最終形成對重癥醫(yī)學(xué)�？谱o(hù)士的專業(yè)技術(shù)水平、臨床服務(wù)能力、教學(xué)能力及科研能力等綜合素質(zhì)的全面評價(jià),為重癥醫(yī)學(xué)�？谱o(hù)士的培訓(xùn)提供客觀的參照標(biāo)準(zhǔn),促使人力資源的有效管理。方法:采用量性與質(zhì)性研究相結(jié)合的方法構(gòu)建重慶市重癥醫(yī)學(xué)�？谱o(hù)士實(shí)踐培訓(xùn)體系。1.通過文獻(xiàn)綜述法了解國內(nèi)外重癥醫(yī)學(xué)專科護(hù)士培訓(xùn)現(xiàn)狀;2.通過量性研究(調(diào)查問卷)和質(zhì)性研究(半結(jié)構(gòu)式訪談)深入的了解重慶市重癥醫(yī)學(xué)�？谱o(hù)士的培訓(xùn)現(xiàn)狀;3.通過兩輪德爾菲法確定指標(biāo)條目,并計(jì)算各指標(biāo)權(quán)重。結(jié)果:(1)共調(diào)查已通過重癥醫(yī)學(xué)專科護(hù)士培訓(xùn)的509名專科護(hù)士,回收有效問卷435份,有效回收率為85.46%。其中女性占95.64%,(年齡在23~48歲),重癥醫(yī)學(xué)科工齡在2~5年占75.63%,大專及以上學(xué)歷占99.31%。參加培訓(xùn)的臨床實(shí)踐時(shí)間均為2個(gè)月,其中對臨床實(shí)踐培訓(xùn)目的明確者占97.47%,對臨床實(shí)踐內(nèi)容清楚者占96.55%,對臨床實(shí)踐效果滿意者占93.57%。(2)兩輪函詢問卷的有效回收率皆為100%,兩輪專家權(quán)威系數(shù)均為0.928,其中判斷依據(jù)均為0.937,熟悉程度也均為0.920;第一輪一、二、三級指標(biāo)的肯德爾協(xié)調(diào)系數(shù)分別為0.236、0.198、0.204,P0.001,第二輪分別為0.283、0.204、0.283,P0.001,最終構(gòu)建的重慶市重癥醫(yī)學(xué)�？谱o(hù)士實(shí)踐培訓(xùn)體系包括6項(xiàng)一級指標(biāo),28項(xiàng)二級指標(biāo)和50項(xiàng)三級指標(biāo)。結(jié)論:研制出的重慶市重癥醫(yī)學(xué)專科護(hù)士實(shí)踐培訓(xùn)體系,具有較好的可操作性,權(quán)重設(shè)置具有較好的科學(xué)性,對各單位進(jìn)行重癥醫(yī)學(xué)�？谱o(hù)士實(shí)踐培訓(xùn)具有指導(dǎo)作用。
[Abstract]:Objective: to establish a practical training system for intensive care nurses in Chongqing on the basis of understanding the current situation of training of intensive care nurses at home and abroad, including the evaluation of training content, curriculum, training effect and so on. Finally, a comprehensive evaluation of the professional technical level, clinical service ability, teaching ability and scientific research ability of the specialized nurses in severe medicine is formed, which provides an objective reference standard for the training of the specialized nurses in severe medicine. To promote the effective management of human resources. Methods: the practical training system of Chongqing specialized nurses in critical medicine was constructed by combining quantitative and qualitative studies. 1. Through the literature review method to understand the domestic and foreign intensive medicine specialized nurses training status; 2. Research on throughput (questionnaire) and qualitative research (semi-structured interview) deeply understand the training status of Chongqing specialized nurses in critical medicine; 3. Through two rounds of Delphi method to determine the index items, and calculate the weight of each index. Results: (1) A total of 509 nurses trained in intensive medicine were investigated, 435 valid questionnaires were collected, and the effective recovery rate was 85.46%. Among them, 95.64% were female (aged 23 ~ 48), 75.63% were in intensive medicine for 2 ~ 5 years, 99.31% were college degree or above. The clinical practice time of training was 2 months. Among them, 97.47 have definite aim of clinical practice training, 96.55555 have clear clinical practice content, 93.577am are satisfied with clinical practice effect. (2) the effective recovery rate of two rounds of questionnaire is 100, and the expert authority coefficient of two rounds is 0.928, in which judgment is made. The basis is 0.937, and the familiarity is 0.920. Second, the Kendall coordination coefficient of the third grade index was 0.236U 0.1980.204g P0.001, and the second round was 0.283n0.2040.283mP0.001respectively. The training system of Chongqing specialized nurses in intensive medicine included 6 first class indexes, 28 second class indexes and 50 third grade indexes. Conclusion: the practical training system of Chongqing specialized nurses in critical medicine has good maneuverability and scientific weight setting. It can guide every unit to carry out practical training of specialized nurses in severe medicine.
