ICU多重耐藥菌目標(biāo)性監(jiān)測與干預(yù)效果分析

【摘要】:目的了解重癥監(jiān)護(hù)病房(ICU)多重耐藥菌(MDRO)的感染現(xiàn)狀,評價目標(biāo)性監(jiān)測的干預(yù)效果。方法采用前瞻性研究方法,選取2014—2015年入住ICU的患者,2014年1—12月為干預(yù)前階段,2015年1—12月為干預(yù)階段,對比分析干預(yù)前后MDRO感染趨勢。結(jié)果干預(yù)前檢出MDRO297株,干預(yù)后檢出217株,除CRPA外,干預(yù)后耐碳青霉烯類鮑曼不動桿菌(CRAB)、耐碳青霉烯類腸桿菌科細(xì)菌(CRE)、耐甲氧西林金黃色葡萄球菌(MRSA)、耐萬古霉素腸球菌(VRE)檢出株數(shù)均較干預(yù)前減少。ICU MDRO感染率由干預(yù)前的7.17‰降至3.88‰,其中CRAB、CRE干預(yù)后感染率低于干預(yù)前,差異有統(tǒng)計(jì)學(xué)意義(均P0.05);綜合、內(nèi)科ICU MDRO感染率分別由干預(yù)前的8.75‰、7.84‰,降至干預(yù)后的4.39‰、2.28‰,兩組比較差異均有統(tǒng)計(jì)學(xué)意義(均P0.05)。采取干預(yù)措施后,接觸隔離醫(yī)囑24 h下達(dá)率、手衛(wèi)生依從率、醫(yī)護(hù)人員知曉率等防控措施的依從性較干預(yù)前顯著提高(均P0.05)。結(jié)論采取目標(biāo)性監(jiān)測和干預(yù)措施,可降低ICU MDRO檢出率。
[Abstract]:Objective to investigate the infection status of (ICU) multidrug resistant bacteria (MDRO) in intensive care unit (ICU) and evaluate the effect of targeted surveillance. Methods prospective study was used to analyze the trend of MDRO infection in patients admitted to ICU from January to December 2014 and from January to December 2015 respectively. Results MDRO297 strains were detected before intervention and 217 strains after intervention, except for CRPA. After intervention, the number of (CRE), resistant to methicillin-resistant Staphylococcus aureus, (MRSA), and vancomycin resistant Enterococcus (VRE) were decreased compared with those before intervention. The infection rate of MDRO was decreased by intervention.
