【摘要】：目的調(diào)查呼吸科護(hù)士在藥物準(zhǔn)備過程中被中斷的相關(guān)因素,以更好地指導(dǎo)護(hù)士安全給藥。方法2015年4月1日-5月15日對本院呼吸科護(hù)士進(jìn)行連續(xù)觀察。觀察臨床護(hù)士數(shù)、藥物準(zhǔn)備護(hù)士數(shù)、臨床患者數(shù)、藥物準(zhǔn)備持續(xù)時間、準(zhǔn)備的藥物數(shù)等。同時采用"藥物準(zhǔn)備過程護(hù)理中斷事件登記表"記錄護(hù)理用藥過程中發(fā)生的中斷情況、中斷次數(shù)、中斷持續(xù)時間、是否離開配藥室、中斷來源及發(fā)生分布、是否導(dǎo)致用藥差錯等。結(jié)果本研究共觀察了13例護(hù)士,平均每名護(hù)士負(fù)責(zé)(11.4±3.9)位患者,藥物準(zhǔn)備的持續(xù)時間(15.6±8.8)min,需準(zhǔn)備的藥物數(shù)量為(46.8±20.3)份。在147次觀察中,有133次觀察到藥物準(zhǔn)備過程被中斷,出現(xiàn)3次中斷的頻率最高,為30.8%,2次中斷占26.3%;中斷持續(xù)時間1min占17.3%,1~4min占44.4%,5min占38.3%;中斷后護(hù)士離開配藥室的占26.3%,離開配藥室的時間1 min占51.4%,≥1 min占48.6%;發(fā)生中斷的原因主要來自患者(20.30%)、家屬(27.07%)、其他護(hù)士(13.53%)、護(hù)士自身(3.76%)、醫(yī)生(6.01%)、管理者(3.01%)、環(huán)境(22.56%)及其他因素(3.76%);在醫(yī)囑處理環(huán)節(jié)發(fā)生46次中斷,占34.59%,藥物配置環(huán)節(jié)發(fā)生33次中斷,占24.81%,給藥執(zhí)行過程中發(fā)生42次中斷,占31.58%,其他環(huán)節(jié)發(fā)生12次中斷,占9.02%。中斷導(dǎo)致的3例用藥錯誤均為錯誤隱患事件,均未導(dǎo)致錯誤發(fā)生�；颊邤�(shù)、藥物準(zhǔn)備總數(shù)、平均準(zhǔn)備的藥物數(shù)及平均每位護(hù)士負(fù)責(zé)的患者數(shù)與中斷次數(shù)及中斷時間呈正相關(guān),P0.05。結(jié)論呼吸科藥物準(zhǔn)備護(hù)理中的被中斷率較高,來源主要為患者及患者家屬,合理控制每位護(hù)士負(fù)責(zé)的患者數(shù)量,對減少中斷率具有重要意義。
[Abstract]:Objective to investigate the factors related to the interruption of respiratory nurses in the course of drug preparation so as to better guide nurses in the safe administration of drugs. Methods Nurses in respiratory department of our hospital were observed continuously from April 1 to May 15, 2015. The number of clinical nurses, the number of drug preparation nurses, the number of clinical patients, the duration of drug preparation and the number of drugs prepared were observed. At the same time, the "Registration form of Nursing interruption events in Drug preparation process" was used to record the interruptions, the number of interruptions, the duration of the interruptions, whether to leave the dispensing room, the source of the interruptions and the distribution of the interruptions. Whether or not to cause drug errors and so on. Results A total of 13 nurses were observed in this study. The average number of patients in charge of each nurse was (11.4 鹵3.9).
