心臟磁共振特征追蹤技術(shù)定量評價肥厚型心肌病患者心肌形變

【摘要】:目的探討心臟磁共振(CMR)特征追蹤(CMR-FT)技術(shù)定量評價肥厚型心肌病(HCM)患者心肌形變的價值。方法對16例HCM患者(HCM組)及18名健康志愿者(正常對照組)采用CMR-FT技術(shù)行CMR檢查。比較兩組間左心室舒張末期容積(LVEDV)、收縮末期容積(LVESV)、左心室射血分數(shù)(LVEF)、左心室質(zhì)量(LVMASS)及左心室心肌整體徑向應變(RS)、環(huán)向應變(CS)的差異。并分析HCM患者各節(jié)段室壁厚度與節(jié)段性RS、CS的相關(guān)性及整體RS、CS與LVEDV、LVESV、LVEF、LVMASS間的相關(guān)性。結(jié)果HCM組的LVMASS高于正常對照組[(133.74±79.13)g vs(76.87±14.15)g,P=0.01]。兩組間LVEDV、LVESV、LVEF差異均無統(tǒng)計學意義(P均0.05)。HCM組心肌整體RS、CS值均明顯低于正常對照組[RS:(27.05±13.35)%vs(40.62±4.92)%,P0.01;CS:(-8.68±5.56)%vs(-20.73±1.56)%,P0.01]。HCM患者各節(jié)段室壁厚度與節(jié)段性RS(r=-0.41,P0.01)、CS(r=0.28,P0.01)間無相關(guān)性;心肌整體RS(r=-0.36、-0.41、0.22、-0.36)、CS(r=0.34、0.10、0.22、0.42)與LVEDV、LVESV、LVEF、LVMASS間無相關(guān)性(P均0.05)。結(jié)論 CMR-FT有助于定量評估HCM患者心肌形變情況。
