復(fù)旦中文版溝通功能分級(jí)系統(tǒng)的信度和效度研究

【摘要】:目的確定復(fù)旦中文版溝通功能分級(jí)系統(tǒng)(CFCS)的信度和效度。方法以2017年5月至2017年10月在復(fù)旦大學(xué)附屬兒科醫(yī)院康復(fù)中心和上海董李鳳美康健學(xué)校中2~18歲腦癱和具有溝通障礙的學(xué)齡前兒童(PCCD)為研究對(duì)象,排除具有嚴(yán)重聽力和視力障礙者和非漢語溝通的家庭。由研究小組多名兒童康復(fù)醫(yī)師和治療師根據(jù)英語版CFCS翻譯、回譯和集體討論形成復(fù)旦中文版CFCS。在言語語言治療師(SLP)進(jìn)行現(xiàn)場CFCS評(píng)級(jí)的同時(shí),照顧者根據(jù)CFCS提供的評(píng)估流程行回顧性評(píng)估,相對(duì)照顧較多的為第一照顧者,較少的為第二照顧者。通過分析SLP和照顧者、照顧者之間的評(píng)估結(jié)果的相關(guān)性,確定CFCS的評(píng)估者間信度。以能力低下兒童評(píng)定量表(PEDI)作為校標(biāo)行效度檢驗(yàn),PEDI評(píng)估者為兒童照顧者或兒童在校的主要任課老師,通過分析SLP的CFCS分級(jí)與PEDI社會(huì)功能項(xiàng)目的尺度化分之間的相關(guān)性,確定CFCS的效度。結(jié)果符合本文納入和排除標(biāo)準(zhǔn)的研究對(duì)象139名兒童,男91例,年齡2.0~15.8(4.7±3.2)歲,2~4歲74例(53.2%),~16歲65例。腦癱兒童80例均行粗大運(yùn)動(dòng)功能分級(jí)系統(tǒng)(GMFCS)評(píng)估,行手功能分級(jí)系統(tǒng)(MACS)評(píng)估47例;PCCD兒童59例。同1名SLP對(duì)139個(gè)納入對(duì)象行CFCS評(píng)估。102/139(73.4%)個(gè)家庭至少有1名照顧者參與了CFCS評(píng)估,48/102(47.1%)個(gè)家庭有2名照顧者參與了CFCS評(píng)估,總體信度SLP與照顧者、第一和二照顧者間CFCS評(píng)級(jí)Kw分別為0.83和0.85。102個(gè)參與了CFCS評(píng)估家庭中55例腦癱兒童,其中20例有2名照顧者參與了CFCS評(píng)級(jí),SLP與照顧者、第一和二照顧者間CFCS評(píng)級(jí)Kw分別為0.88和0.86;PCCD 47例兒童,其中28例有2名照顧者參與了CFCS評(píng)級(jí),SLP與照顧者、第一和二照顧者間CFCS評(píng)級(jí)Kw分別為0.85和0.82。2~4歲兒童的CFCS評(píng)價(jià)在SLP和照顧者間的信度為Kw=0.77,照顧者間的信度為Kw=0.90;~16歲兒童的CFCS評(píng)級(jí)在SLP和照顧者間的信度為Kw=0.87,照顧者間的信度為Kw=0.74。139例納入對(duì)象SLP的CFCS級(jí)別與PEDI社會(huì)功能項(xiàng)目的尺度化分之間的相關(guān)系數(shù)為-0.81。結(jié)論復(fù)旦中文版CFCS在腦癱兒童和PCCD中均具有良好的評(píng)價(jià)者間信度和平行效度,可以有效地對(duì)這些兒童進(jìn)行溝通功能分級(jí)。
[Abstract]:Objective to determine the reliability and validity of Fudan Chinese version communication function classification system (CFCS). Methods from May 2017 to October 2017, (PCCD) of preschool children with cerebral palsy and communication disorder were studied in the Rehabilitation Center of Pediatrics Hospital affiliated to Fudan University and Shanghai Dong Li Fengmei Health School. Exclude families with severe hearing and visual impairment and non-Chinese communication. A group of child rehabilitation doctors and therapists translated, translated and discussed the Chinese version of Fudan according to the English version of CFCS. At the same time the speech therapists (SLP) rated the CFCS in the field, the caretakers evaluated retrospectively according to the evaluation process provided by CFCS, and the first caregivers and the second caregivers were more cared for than those for the second caregivers. The reliability of CFCS was determined by analyzing the correlation between the evaluation results of SLP and caregivers. Using (PEDI) as the school standard and validity test, the paediatric caregivers or the main teachers in the school were used to analyze the correlation between the CFCS grading of SLP and the scale of PEDI social function items, and analyzed the correlation between CFCS classification of SLP and the scale of PEDI social function items by analyzing the correlation between the CFCS classification of SLP and the scale of PEDI social function items. To determine the validity of CFCS. Results one hundred and thirty-nine children (91 males), aged from 2.0 to 15.8 (4.7 鹵3.2) years old, aged from 4 to 4 years old (53.2%) were enrolled and excluded. 65 children aged 16 years were enrolled in this study. All the 80 children with cerebral palsy were assessed with (GMFCS), and 47 children with cerebral palsy were assessed by (MACS). With one SLP, 139 subjects were evaluated with CFCS. 102 / 139 (73.4%). At least one caretaker took part in the CFCS evaluation 48 / 102 (47.1%). Two caregivers participated in the CFCS evaluation. The CFCS ratings between the first and second caregivers were 0.83 and 0.85.102, respectively. 55 children with cerebral palsy were assessed by CFCS, 20 of whom were involved in CFCS rating. The CFCS rating between the first and second caregivers was 0.88 and 0.86 respectively, among which 2 caregivers were involved in the CFCS rating system. CFCS rating between first and second caregivers Kw was 0.85 and 0.82.2 years old, respectively. The reliability of CFCS between SLP and carer was 0.77, between caretaker and caretaker was 0.90 CFCS rating between SLP and caretaker was Kw0.87, between caretaker and caretaker, the reliability of CFCS between caretaker and caretaker was 0.87. The correlation coefficient between the CFCS level of SLP and the scale of social function items of PEDI was -0.81. Conclusion the Chinese version of Fudan CFCS has good reliability and parallel validity in children with cerebral palsy and PCCD, and can effectively classify the communication function of these children.
