【摘要】：目的 評估Valsalvas閉氣加壓MRI掃描法對咽鼓管功能診斷價值。方法 選取慢性鼻竇炎合并中耳乳突炎者30例(研究組)和健康志愿者20例(對照組)為研究對象。兩組均于Valsalvas加壓前、后進行咽鼓管MRI掃描。觀察Valsalvas加壓前后咽鼓管咽口、深部咽鼓管的形態(tài)特點,對其形態(tài)進行分類,Ⅰ類:潛在間隙型咽鼓管、淺三角形咽口;Ⅱ類:小喇叭狀咽口及裂隙狀咽鼓管、管狀咽鼓管。測量咽口寬度、深度及深部咽鼓管寬度。結果 對照組40個咽口中,Valsalvas加壓前有Ⅰ類咽鼓管13個,Ⅱ類咽鼓管27個,Valsalvas加壓后有Ⅰ類2個,Ⅱ類咽口38個,與加壓前形態(tài)差異有統(tǒng)計學意義(χ2=5.02,P0.05)。研究組60個咽鼓管中,閉氣前Ⅰ類咽口38個,Ⅱ類22個,閉氣后Ⅰ類咽口39個,Ⅱ類21個;Valsalvas加壓前后其形態(tài)差異無統(tǒng)計學意義(χ2=0.85,P0.05)。Valsalvas加壓后研究組咽鼓管咽口平均寬徑(3.95±0.37)mm,對照組咽鼓管咽口平均寬徑(4.78±0.58)mm,兩組間差異有統(tǒng)計學意義(t=1.95,P0.05)。結論 Valsalvas加壓MRI可以有效評價咽鼓管功能。
[Abstract]:Objective to evaluate the value of valsalvas gas closure and compression MRI in diagnosis of eustachian tube function. Methods 30 cases of chronic sinusitis complicated with middle ear and mastoiditis (study group) and 20 healthy volunteers (control group) were selected. Both groups underwent MRI scanning of eustachian tube before Valsalvas pressure. The morphological characteristics of the pharyngeal and deep eustachian tubes before and after Valsalvas pressure were observed and classified into three categories: latent interstitial eustachian tubes, shallow triangular pharyngeal orifices, small trumpet pharyngitis and fissured eustachian tubes, and tubular eustachian tubes.
