【摘要】：目的了解原發(fā)性肝癌男性患者真實(shí)心理困境,全面分析原發(fā)性肝癌男性患者心理困境及其相關(guān)因素,幫助護(hù)理人員充分評(píng)估其心理歷程,為采取積極有效措施促進(jìn)其身心康復(fù),提高患者治療效果和應(yīng)對(duì)能力提供依據(jù)。方法本研究采用質(zhì)性研究中的現(xiàn)象學(xué)研究方法,按照目的抽樣法,遵循“信息飽和”原則,在山東省某三級(jí)甲等醫(yī)院消化科選取10名原發(fā)性肝癌男性患者為研究對(duì)象。由研究者本人實(shí)施并根據(jù)研究目的制定訪談提綱,對(duì)被試進(jìn)行半結(jié)構(gòu)式深度訪談,探討并記錄被試真實(shí)的心理體驗(yàn)。研究者反復(fù)仔細(xì)閱讀訪談?dòng)涗?同時(shí)結(jié)合訪談情景的回憶,選用Colaizzi內(nèi)容分析法對(duì)訪談所得資料予以整理及分析,提煉主題,并進(jìn)行討論分析。結(jié)果資料分析發(fā)現(xiàn),患者因文化程度、教育背景、生活經(jīng)歷等個(gè)體差異而表現(xiàn)各不相同,但面對(duì)相同的應(yīng)激有共同的心理體驗(yàn)和反應(yīng),面對(duì)很多相同的抉擇和困境。經(jīng)過對(duì)資料的反復(fù)分析和整理,提煉的原發(fā)性肝癌男性患者心理困境的主題及亞主題如下：1.主題一獲知診斷后的心理困境亞主題：(1)震驚和否認(rèn)；(2)恐懼；(3)憤怒；(4)悔恨；(5)災(zāi)難化反應(yīng)2.主題二治療過程中的心理困境亞主題：(1)焦慮、抑郁；(2)無奈；(3)疾病不確定感3.主題三家庭因素所致的心理困境亞主題：(1)贍養(yǎng)父母能力缺失；(2)撫養(yǎng)子女責(zé)任受阻；(3)經(jīng)濟(jì)壓力；(4)家庭角色轉(zhuǎn)為受照顧者；(5)家庭完整性受損4.主題四社會(huì)因素所致的心理困境亞主題：(1)社會(huì)地位改變；(2)社交障礙；(3)工作無法勝任5.主題五心理困境改善需求亞主題：(1)對(duì)疾病知情權(quán)的需求；(2)求醫(yī)需求；(3)疾病知識(shí)需求；(4)心理幫助需求結(jié)論1.原發(fā)性肝癌男性患者的心理體驗(yàn)復(fù)雜多樣,每個(gè)個(gè)體應(yīng)對(duì)方式存在差異,心理困境表現(xiàn)形式因人而異,心理困境改善的需求也相應(yīng)的存在個(gè)體差異。2.醫(yī)護(hù)人員要重視每位患者的不同的心理困境,從患者的各個(gè)方面的需求考慮,針對(duì)每位患者制定出個(gè)性化、人性化的護(hù)理措施,提供針對(duì)性的幫助,改善心理困境。
[Abstract]:Objective to understand the true psychological predicament of male patients with primary liver cancer (HCC), to analyze the psychological dilemma of male patients with PHC and its related factors, to help nurses to evaluate their psychological course, and to take active and effective measures to promote their physical and mental recovery. To provide evidence for improving the therapeutic effect and coping ability of patients. Methods in this study, 10 male patients with primary liver cancer were selected from the digestive department of a Grade 3A hospital in Shandong Province, according to the objective sampling method and the principle of "information saturation". According to the purpose of the study, the researcher conducted a semi-structured in-depth interview to explore and record the real psychological experience of the subjects. The researcher repeatedly read the interview records carefully, combined with the memory of the interview scene, selected Colaizzi content analysis method to collate and analyze the interview data, refine the topic, and carry out discussion and analysis. Results the data showed that the patients were different because of their educational background, educational background and personal experience, but they had the same psychological experience and reaction in the face of the same stress, and many of the same choices and dilemmas. After repeated analysis and collation of the data, the theme and subthemes of the psychological distress of male patients with primary liver cancer are as follows: 1. Topics 1: (1) shock and denial; (2) fear; (3) anger; (4) regret; (5) disaster response. Subthemes of psychological distress in the course of treatment: (1) anxiety, depression, (2) helplessness, (3) feeling of uncertainty. The sub-themes of psychological distress caused by family factors are: (1) lack of ability to support parents; (2) hindered responsibility for raising children; (3) financial pressure; (4) family role to be taken care of; (5) impaired family integrity. Subject 4: subthemes of psychological distress caused by social factors: (1) social status change; (2) social handicap; (3) work incompetence. Topic 5: the subthemes of improving the need for psychological distress: (1) the need for the right to know the disease; (2) the need for medical treatment; (3) the need for disease knowledge; (4) the conclusion of the need for psychological help. The psychological experience of male patients with primary liver cancer is complex and diverse, each individual coping style is different, the manifestation of psychological distress varies from person to person, and the need for improvement of psychological predicament also has individual differences. 2. The medical staff should pay attention to the different psychological predicament of each patient, consider the needs of each patient, formulate individualized and humanized nursing measures for each patient, provide targeted help and improve the psychological predicament.
