【摘要】：目的了解經(jīng)外周靜脈置入中心靜脈導(dǎo)管(PICC)置管腫瘤化療患者對(duì)居家維護(hù)的接受程度及其影響因素,為更多帶管回家患者接受PICC居家維護(hù)提供依據(jù)。方法自行設(shè)計(jì)調(diào)查問卷,采用方便抽樣方法,對(duì)160例PICC置管患者進(jìn)行問卷調(diào)查。結(jié)果接受居家維護(hù)患者74例,未接受86例,付費(fèi)方式、行動(dòng)能力、住所距維護(hù)點(diǎn)距離、較方便的導(dǎo)管維護(hù)方式4個(gè)因素對(duì)PICC置管的腫瘤化療患者居家維護(hù)接受程度的影響差異有統(tǒng)計(jì)學(xué)意義(OR值分別為2.180、2.959、3.688、2.293),其他調(diào)查因素對(duì)居家維護(hù)接受程度的影響差異無統(tǒng)計(jì)學(xué)意義(P0.05);患者接受與不接受居家維護(hù)的原因進(jìn)一步分析顯示,接受居家維護(hù)的患者中83.78%認(rèn)為居家維護(hù)比返院維護(hù)省時(shí)、省錢,40.54%認(rèn)為居住地離維護(hù)點(diǎn)太遠(yuǎn),返維護(hù)點(diǎn)維護(hù)不方便,不接受居家維護(hù)患者中75.58%認(rèn)為住所離維護(hù)點(diǎn)近,返院維護(hù)方便。結(jié)論 PICC居家維護(hù)接受程度呈較低水平,影響患者接受PICC居家維護(hù)的主要因素為住所距維護(hù)點(diǎn)距離、付費(fèi)方式、行動(dòng)能力和較方便的導(dǎo)管維護(hù)方式,建議向基層醫(yī)院推廣PICC維護(hù)技術(shù),進(jìn)一步建立、完善PICC社區(qū)維護(hù)網(wǎng)絡(luò);同時(shí)制定標(biāo)準(zhǔn)化、專業(yè)化、管理規(guī)范化的PICC居家維護(hù)模式,為部分家住偏遠(yuǎn)地區(qū)、行動(dòng)不便的患者提供新的選擇。
[Abstract]:Objective to understand the degree of acceptance of home maintenance and its influencing factors by peripherally inserted central venous catheter (PICC) catheterization chemotherapy patients for home maintenance, and to provide a basis for more home maintenance patients with PICC home maintenance. Methods a self-designed questionnaire was designed and a convenient sampling method was used to investigate 160 patients with PICC catheterization. 74 cases of home maintenance patients were accepted, 86 cases were not accepted, the payment method, the ability of action, the distance from the distance to the maintenance point of the residence and the convenient way of maintenance of the catheter were statistically significant (OR value 2.180,2.959,3.688,2.293) for the home maintenance acceptance of the tumor chemotherapy patients with PICC catheterization (OR value was 2.180,2.959,3.688,2.293 respectively). There was no significant difference in the effect of nursing acceptance (P0.05). Further analysis of the reasons for the patient's acceptance and non acceptance of home maintenance showed that 83.78% of the patients who received home maintenance showed that home maintenance was less expensive than the return hospital, 40.54% thought the residence was too far from the maintenance point, the maintenance point was inconvenient, and the home maintenance was not accepted. 75.58% of the people believed that the residence was close to the maintenance point, and the maintenance of home maintenance was convenient. Conclusion the degree of home maintenance acceptance of PICC is low. The main factors that affect the patient's home maintenance of PICC are the distance from the distance to the maintenance point of the residence, the way of paying, the ability of action and the convenient maintenance mode of the catheter. It is suggested that the PICC maintenance technology should be popularized to the primary hospital. Establish, improve the PICC community maintenance network, and establish a standardized, professional, standardized management of the PICC home maintenance mode, for some families living in remote areas, to provide new options for the inconvenient patients.
