【摘要】：目的探討MRI對(duì)胎兒胎糞性腹膜炎的診斷價(jià)值。方法回顧性分析我院7胎經(jīng)手術(shù)及病理證實(shí)為胎糞性腹膜炎的胎兒產(chǎn)前MRI影像特征。結(jié)果 6胎可見(jiàn)大量游離腹腔積液,腸管漂浮于中腹部,小腸聚攏糾集;1胎表現(xiàn)為腹部巨大囊性包塊,腸管受壓移位、不均勻擴(kuò)張。4胎顯示結(jié)、直腸細(xì)小或胎糞充盈不佳。2胎伴雙側(cè)睪丸鞘膜積液。3例孕婦羊水量多。產(chǎn)后1例患兒于術(shù)中突發(fā)心率下降搶救無(wú)效死亡,1例術(shù)后合并嚴(yán)重肺水腫死亡,余5例預(yù)后良好。結(jié)論胎糞性腹膜炎的產(chǎn)前MRI具有特征性表現(xiàn),可為產(chǎn)后治療及評(píng)估預(yù)后提供重要依據(jù)。
[Abstract]:Objective to evaluate the diagnostic value of MRI in fetal meconium peritonitis. Methods the prenatal MRI features of 7 cases of meconium peritonitis proved by operation and pathology in our hospital were analyzed retrospectively. Results A large number of free celiac effusion was found in 6 fetuses, the intestinal tube was floating in the middle abdomen, the small intestine gathered and gathered in 1 fetus, the abdominal giant cystic mass was found in one fetus, the intestinal tube was displaced under pressure, and the intestinal tube expanded inhomogeneously. 4 fetuses showed nodule. Small rectum or poor meconium filling. 2 fetus with bilateral testicular hydrocele. 3 cases of amniotic fluid. One case died of sudden heart rate drop during operation and 1 case died of severe pulmonary edema after operation. The other 5 cases had a good prognosis. Conclusion Prenatal MRI features of meconium peritonitis can provide important evidence for postpartum treatment and prognosis evaluation.
