【摘要】：目的了解西安地區(qū)住院患者與健康人群Rh血型分布特征,探討Rh抗原分型檢測在臨床輸血安全中的意義。方法采用血清學(xué)方法對10 898例住院患者和5 121例健康人做Rh抗原分型檢測,并對住院患者進(jìn)行不規(guī)則抗體篩查和鑒定。結(jié)果住院患者與健康人群的RhD陽性表型分布頻率差異有統(tǒng)計學(xué)意義,并且在構(gòu)成比中,住院患者表型Dccee(0.42%)DCc EE(0.12%),而健康人DCc EE(0.33%)Dccee(0.21%);在RhD陽性患者中,Rh表型以CCee和Cc Ee為優(yōu)勢表型,在RhD陰性患者中,Rh表型以ccee和Ccee為優(yōu)勢表型;Rh 5種抗原的陰性率由高到低依次為E(51.24%)c(41.42%)C(12.04)e(7.82%)D(0.83%);在檢出的37例同種抗體中,Rh抗體28例,占75.7%,其中抗-E17例,抗-c E4例,抗-D3例,抗-Ce2例,抗-C1例,抗-c1例。結(jié)論應(yīng)用Rh抗原分型檢測技術(shù),選擇與患者Rh表型相匹配的獻(xiàn)血員紅細(xì)胞輸注,可在一定程度上減少同種抗體的產(chǎn)生,降低輸血不良反應(yīng)的發(fā)生率。
[Abstract]:Objective to investigate the distribution characteristics of Rh blood group in inpatients and healthy people in Xi'an, and to explore the significance of Rh antigen typing in clinical transfusion safety. Methods Rh antigen typing was performed in 10 898 hospitalized patients and 5 121 healthy persons by serological method, and irregular antibodies were screened and identified. Results the frequency of RhD positive phenotypic distribution was significantly different between the inpatients and the healthy people. In the composition ratio, the phenotypic phenotypes of Dccee(0.42%)DCc were 0.12 in the inpatients, 0.33 in the healthy subjects, 0.21 in the healthy subjects, and the predominant phenotypes of the Rh phenotypes in the RhD positive patients were CCee and Cc-Ee. In RhD negative patients, the negative rate of Rh 5 antigens with ccee and Ccee as the dominant phenotypes was 51.242.The negative rate of Rh was 41.42 and 41.42%, and 28 of the 37 homoantibodies were detected, accounting for 75.7E4, including anti-E17, anti-C, anti--D3, anti-Ce2, and anti-CE2, respectively, in 37 patients with RhD negative phenotypes, the positive rate of Rh antibodies was 75.7%, including anti-E 17 cases, anti-C cases, anti--D3 cases, anti-E 2 cases, anti-De 3 cases, anti-ce 2 cases, and the negative rates of Rh antigens were 75.7E4%, 75.7E4%, 75.7E4% and 75.7E4% respectively. Anti-C1 and anti-c were found in 1 case. Conclusion Rh antigen typing technique and Rh matching Rh phenotype matching red blood cell transfusion can reduce the generation of homologous antibody and decrease the incidence of adverse reaction of blood transfusion to a certain extent.
