【摘要】：目的探討基于Cox健康行為互動(dòng)模式的健康管理對(duì)社區(qū)糖尿病患者糖尿病相關(guān)生活方式、臨床指標(biāo)及滿意度的影響。方法本研究采用類試驗(yàn)研究的方法,通過(guò)目的抽樣法,選取某三級(jí)甲等醫(yī)院內(nèi)分泌科2015年3月至7月住院治療、病情穩(wěn)定出院且分屬大小、規(guī)模相仿的2個(gè)社區(qū)衛(wèi)生服務(wù)中心(蘭州市張掖路廣武門(mén)社區(qū)衛(wèi)生服務(wù)中心和團(tuán)結(jié)新村社區(qū)衛(wèi)生服務(wù)中心)的2型糖尿病患者作為研究對(duì)象。對(duì)所有研究對(duì)象進(jìn)行基線調(diào)查后按其所屬的社區(qū)衛(wèi)生服務(wù)中心分為試驗(yàn)組和對(duì)照組。對(duì)照組應(yīng)用常規(guī)健康管理模式進(jìn)行管理,試驗(yàn)組進(jìn)行以Cox健康行為互動(dòng)模式為指導(dǎo)的健康管理。測(cè)評(píng)干預(yù)0、6、12個(gè)月兩組患者的糖尿病相關(guān)生活方式、腰臀比(WHR)、體質(zhì)指數(shù)(BMI)、空腹血糖(FPG)、糖化血紅蛋白(HbA1c)以及干預(yù)12個(gè)月患者對(duì)健康管理服務(wù)的滿意度,與傳統(tǒng)健康管理模式對(duì)比,評(píng)價(jià)Cox健康行為互動(dòng)模式在在社區(qū)糖尿病患者健康管理中的應(yīng)用效果。結(jié)果(1)根據(jù)診斷、納入、排除標(biāo)準(zhǔn),本研究共納入110例2型糖尿病患者,試驗(yàn)組和對(duì)照組各55例,經(jīng)過(guò)為期12個(gè)月的健康管理后,試驗(yàn)組失訪4例,剩余51例,對(duì)照組患者失訪5例,剩余50例,最終納入101例2型糖尿病患者進(jìn)行統(tǒng)計(jì)分析。(2)干預(yù)前,試驗(yàn)組和對(duì)照組之間的一般資料、糖尿病相關(guān)生活方式得分及臨床指標(biāo)等方面的比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),具有可比性。(3)干預(yù)6個(gè)月和12個(gè)月,兩組患者在飲食結(jié)構(gòu)、尼古丁依賴評(píng)分、運(yùn)動(dòng)評(píng)分、WHR、FPG、HbA1c等方面均有所改善,且試驗(yàn)組明顯優(yōu)于對(duì)照組,差異均具有統(tǒng)計(jì)學(xué)意義(P0.05);而兩組患者的飲酒評(píng)分及BMI在干預(yù)后各時(shí)間點(diǎn)均無(wú)明顯改善(P0.05)。(4)干預(yù)12個(gè)月后,試驗(yàn)組患者對(duì)社區(qū)健康管理服務(wù)的滿意度評(píng)分明顯高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論基于Cox健康行為互動(dòng)模式的健康管理在社區(qū)2型糖尿病患者糾正不合理的飲食結(jié)構(gòu)、吸煙、運(yùn)動(dòng)不足等不良生活習(xí)慣,改善WHR和血糖控制水平方面發(fā)揮了積極作用,有利于提高患者滿意度,為社區(qū)2型糖尿病患者健康管理提供了可參考模式。
[Abstract]:Objective to investigate the effect of health management based on Cox health behavior interaction model on diabetes related lifestyle, clinical indexes and satisfaction in community diabetic patients. Methods by using the method of class experiment and objective sampling, we selected the department of Endocrinology in a Grade 3A hospital from March to July of 2015. The patients were discharged stably and were in different size. Patients with type 2 diabetes mellitus from two similar community health service centers (Zhangye Lu Guang Wu Men Community Health Service Center and United New Village Community Health Service Center) were studied. After baseline investigation, all subjects were divided into trial group and control group according to their community health service centers. The normal health management model was used in the control group, and the health management guided by the Cox health behavior interaction model was used in the experimental group. Diabetes related lifestyle, WHR, BMI, FBG, HbA1c) were measured in both groups at 6 and 12 months after intervention, and compared with the traditional health management model. To evaluate the effect of Cox health behavior interaction model in community diabetes health management. Results 1) according to the criteria of diagnosis, inclusion and exclusion, 110 patients with type 2 diabetes were included in this study, 55 cases in each group and 55 cases in control group. After 12 months of health management, 4 cases were lost in the trial group and 51 cases in the remaining group. There were 5 cases in the control group and 50 cases in the control group, and 101 patients with type 2 diabetes were included in the study group. Before the intervention, the general data, the scores of diabetes related lifestyle and the clinical indexes were compared between the trial group and the control group. There was no significant difference between the two groups in dietary structure, nicotine dependence score, exercise score and so on, and the experimental group was significantly better than the control group in dietary structure, nicotine dependence score, exercise score and so on. The difference was statistically significant (P 0.05), but the scores of alcohol consumption and BMI were not significantly improved at each time point after intervention. After 12 months of intervention, the satisfaction scores of the patients in the trial group were significantly higher than those in the control group. The difference was statistically significant (P 0.05). Conclusion the health management based on Cox health behavior interaction model plays an active role in correcting unreasonable diet structure, smoking, lack of exercise, and improving the level of WHR and blood sugar control in community type 2 diabetes patients. It is beneficial to improve patient satisfaction and provide a reference model for the health management of type 2 diabetes patients in community.
