MR擴散加權(quán)成像定量化分析在預(yù)測小肝癌分化程度的價值

【摘要】:目的探討磁共振擴散加權(quán)成像(DWI)定量化分析在不同分化程度小肝癌(sHCC)的診斷價值。方法將經(jīng)病理證實的36例sHCC納入研究,所有患者術(shù)前均行常規(guī)MRI和DWI(b=0,600 s/mm~2)檢查。測量瘤灶的DWI-ADC值,信號強度(SI)值及信號相對比值(RCR),并探討其與sHCC不同分化程度的關(guān)系。結(jié)果 36例sHCC患者,包括高分化組(n=20)、中分化組(n=10)和低分化組(n=6)。高、中和低分化組的sHCC病灶的平均ADC值分別為(1.46±0.68)×10~(-3)mm~2/s、(1.32±0.56)×10~(-3)mm~2/s和(1.13±0.63)×10~(-3)mm~2/s,各組間差異具有統(tǒng)計學意義。高、中和低分化組的sHCC病灶的平均SI值分別為87.73±4.82、72.73±8.04和89.13±5.98;而平均RCR值分別為1.37±0.13、1.57±0.13和1.71±0.10。SI值高、低分化組間差異無統(tǒng)計學意義,而RCR值在各組間差異均有統(tǒng)計學意義。ROC曲線顯示ADC值較SI值更有助于區(qū)分高分化與中低分化sHCC,其曲線下面積為0.964;當ADC值≥1.39×10~(-3)mm~2/s時,判斷高分化HCC的敏感性為85%,特異性為93.7%。結(jié)論基于DWI技術(shù)的ADC值能有效判斷不同分化程度s HCC,而SI值提供相對有限的價值。
[Abstract]:Objective to evaluate the diagnostic value of diffusion weighted Mr imaging (DWI) in the diagnosis of small hepatocellular carcinoma (HCC) with different differentiation. Methods 36 cases of sHCC confirmed by pathology were included in the study. All patients were examined by routine MRI and DWI(b=0600 s / mmm-2 before operation. The DWI-ADC value, signal intensity and signal relative ratio of tumor focus were measured, and the relationship between them and the differentiation degree of sHCC was discussed. Results 36 patients with sHCC, including well-differentiated group, moderately differentiated group, and poorly differentiated group, were included. The mean ADC values of sHCC lesions in the high, middle and low differentiation groups were 1.46 鹵0.68) 脳 10 ~ (-3) / 2 ~ (-1) 10~(-3)mm~2/s and 1.13 鹵0.63) 脳 10 ~ (10) ~ (-3) mm ~ (-1) / s, respectively. The difference was statistically significant among the three groups. The average SI values of sHCC lesions were 87.73 鹵4.82 鹵8.04 and 89.13 鹵5.98 in the high, middle and low differentiation groups, respectively, while the average RCR values were 1.37 鹵0.131.57 鹵0.13 and 1.71 鹵0.10.SI, respectively. There was no significant difference between the low differentiation group and the low differentiation group. The difference of RCR value among all groups was statistically significant.
