2型糖尿病患者下肢動脈血管回聲跟蹤技術檢測指標與血脂及血糖的相關性

【摘要】:目的應用血管回聲跟蹤技術檢測2型糖尿病患者足背動脈及乆動脈彈性指標,探討其與血脂及血糖的相關性。方法 2型糖尿病患者40例(觀察組),體檢健康者40例(對照組),檢測2組空腹血糖(fasting plasma glucose,FPG)、糖化血紅蛋白(glycosylated hemoglobin,HbA_(1c))、總膽固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、高密度脂蛋白膽固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白膽固醇(low density lipoproteincholesterol,LDL-C)水平;應用血管回聲跟蹤技術檢測2組足背動脈、乆動脈僵硬度(stiffness parameterβ,β)、血管壓力應變彈性系數(shù)(pressure-strain elasticity coefficient,E_ρ)、順應性(arterial compliance,AC)、脈搏波傳導速度(pulse wave velocityβ,PWVβ);Pearson相關分析2型糖尿病患者足背動脈和乆動脈彈性指標與FPG、HbA_(1c)、TC、TG、HDL-C、LDL-C的相關性。結果觀察組FPG[(8.01±1.95)mmol/L]、HbA_(1c)[(8.66±1.64)mmol/L]、TC[(5.28±0.84)mmol/L]、TG[(1.96±0.89)mmol/L]、LDL-C[(3.11±0.65)mmol/L]高于對照組[(5.35±0.62)、(5.19±0.29)、(4.77±0.21)、(1.54±0.11)、(2.82±0.30)mmol/L](P0.05);觀察組左、右側足背動脈β(14.41±5.06,14.68±4.80)、E_ρ[(198.35±72.89)、(199.82±67.24)kPa]、PWVβ[(8.46±1.66)、(8.52±1.51)m/s]高于對照組[(10.99±4.28)、(11.37±4.25);(137.09±52.38)、(141.27±54.82)kPa;(7.05±1.37)、(7.18±1.37)m/s](P0.05),AC值[(0.080±0.030)、(0.082±0.040)mm~2/kPa]低于對照組[(0.104±0.043)、(0.103±0.042)mm~2/kPa](P0.05);觀察組乆動脈左、右側β(14.21±6.08、14.38±5.86)、E_ρ[(200.35±96.12)、(200.60±88.66)kPa]、PWVβ[(8.51±2.00)、(8.37±1.79)m/s]高于對照組[9.85±4.78、10.29±4.20;(122.77±63.00)、(127.15±52.10)kPa;(6.64±1.62)、(6.79±1.79)m/s](P0.05),AC值[(0.30±0.14)、(0.34±0.18)mm~2/kPa]低于對照組[(0.52±0.22)、(0.49±0.20)mm~2/kPa](P0.05);2型糖尿病患者足背動脈、乆動脈彈性指標β、E_ρ、PWVβ與FPG、HbA_(1c)、TC、LDL-C呈正相關,足背動脈AC與FPG、TC呈負相關,乆動脈AC與HbA_(1c)、TC呈負相關。結論 2型糖尿病患者下肢動脈粥樣硬化主要與FPG、HbA_(1c)、TC、LDL-C有關,血管回聲跟蹤技術可快速、無創(chuàng)地檢查下肢小動脈彈性,及時發(fā)現(xiàn)血管彈性功能受損情況。
[Abstract]:Objective to investigate the relationship between vascular echo tracking and blood lipids and blood glucose in patients with type 2 diabetes mellitus. Methods Forty patients with type 2 diabetes mellitus (observation group) and 40 healthy persons (control group) were examined for fasting plasma glucose GG, glycosylated hemoglobinated hemoglobin, total cholesterol, triacylglycerol TGG, high density lipoprotein cholesterol high density in the two groups of patients with type 2 diabetes mellitus (control group, n = 40), and glycosylated hemoglobinized hemoglobin (HbAG), total cholesterol (TC), triacylglycerol (TG), high density lipoprotein cholesterol (HDL-C) in the two groups. Low density protein cholesterol (LDL-C); Vascular echo tracking technique was used to detect the dorsal foot artery in two groups.
