【摘要】：研究目的:本研究旨在循證的基礎(chǔ)上,建立科學(xué)、實(shí)用的成人危重患者院內(nèi)轉(zhuǎn)運(yùn)規(guī)范,為各級(jí)醫(yī)院成人危重患者院內(nèi)轉(zhuǎn)運(yùn)的實(shí)施及安全管理提供參考依據(jù)。研究方法:本研究根據(jù)循證護(hù)理研究方法制定嚴(yán)密的檢索策略,采用美國(guó)霍普金斯證據(jù)等級(jí)與質(zhì)量評(píng)價(jià)方法對(duì)檢索到的相關(guān)文獻(xiàn)進(jìn)行等級(jí)與質(zhì)量評(píng)定,總結(jié)出成人危重患者院內(nèi)轉(zhuǎn)運(yùn)相關(guān)條目,經(jīng)專(zhuān)家小組討論后初步擬定適合中國(guó)國(guó)情的成人危重患者院內(nèi)轉(zhuǎn)運(yùn)規(guī)范,采用德?tīng)柗品?按照5分級(jí)Likert標(biāo)度法,對(duì)初步形成的條目的重要性、可行性進(jìn)行評(píng)價(jià)。由浙江省各地區(qū)的50名專(zhuān)家進(jìn)行函詢論證,確立符合中國(guó)國(guó)情的成人危重患者院內(nèi)轉(zhuǎn)運(yùn)規(guī)范,運(yùn)用專(zhuān)家積極系數(shù)、權(quán)威系數(shù)、變異系數(shù)、協(xié)調(diào)系數(shù)等指標(biāo),對(duì)專(zhuān)家函詢的可靠性和代表性進(jìn)行檢驗(yàn)。研究結(jié)果:構(gòu)建了包含組織管理、教育培訓(xùn)、物品管理及轉(zhuǎn)運(yùn)流程四部分的成人危重患者院內(nèi)轉(zhuǎn)運(yùn)規(guī)范。兩輪專(zhuān)家函詢的積極系數(shù)分別為98%和91.83%(70%),專(zhuān)家的權(quán)威系數(shù)分別為0.87和0.90(≥0.7),兩輪專(zhuān)家函詢后各條目重要性的變異系數(shù)為0.03～0.24,可行性變異系數(shù)為0.00～0.27(0.30)。研究結(jié)論:構(gòu)建了包含組織管理、教育培訓(xùn)、物品管理及轉(zhuǎn)運(yùn)流程四部分的成人危重患者院內(nèi)轉(zhuǎn)運(yùn)規(guī)范。專(zhuān)家函詢的積極系數(shù)、權(quán)威系數(shù)、變異系數(shù)等結(jié)果證明專(zhuān)家函詢代表性和可信度高。
[Abstract]:Objective: the purpose of this study was to establish a scientific and practical standard of hospital transport for adult critical patients on the basis of evidence, and to provide reference for the implementation and safety management of hospital transport for adult critical patients in all levels of hospitals. Methods: according to the method of evidence-based nursing research, a rigorous retrieval strategy was established in this study. Hopkins evidence rating and quality evaluation method was used to evaluate the relevant literature. The related items of in-hospital transport of adult critical patients were summarized. After the discussion of the expert group, the standard of in-hospital transport of adult critical patients suitable for China was drawn up preliminarily. The Delphi method was adopted, and the 5-grade Likert scaling method was used. Evaluate the importance and feasibility of the preliminary items. With 50 experts from various regions of Zhejiang Province, we have conducted a letter and argumentation to establish the standard of in-hospital transportation of adult critical patients in accordance with China's national conditions, and to apply the positive coefficient of experts, the authoritative coefficient, the coefficient of variation, the coefficient of coordination, and so on. Test the reliability and representativeness of expert inquiry. Results: four parts, including organization management, education and training, goods management and transportation process, were constructed for hospital transportation of adult critical patients.
