【摘要】：目的探討MRS技術在非海馬硬化顳葉癲癇患者中的應用價值。方法回顧性分析于我院診治為非海馬硬化的單側顳葉癲癇患者23例(病例組),選取年齡與之相匹配的20名正常志愿者作為對照組,行常規(guī)MR平掃及MRS檢查,分析顳葉癲癇患者患側海馬、健側海馬及對照組海馬N-乙酰天門冬氨酸(NAA)/肌酸(Cr)、膽堿(Cho)/Cr值的差異;同時評價病例組中接受手術治療的11例患者的病理及手術結果。結果病例組患側海馬NAA/Cr值較健側海馬(t=-7.97,P0.001)及對照組海馬降低(t=-9.96,P0.001),病例組健側海馬NAA/Cr值與對照組比較差異無統(tǒng)計學意義(t=-1.21,P=0.12)。病例組患側海馬Cho/Cr與健側(t=0.50,P=0.31)及對照組海馬(t=-0.59,P=0.28)差異無統(tǒng)計學意義。11例行致癇側顳葉切除患者的病理結果顯示小范圍少量的神經(jīng)元損傷,神經(jīng)膠質增生不明顯。結論MRS有助于臨床對非海馬硬化顳葉癲癇患者的致癇灶進行定側及定位。
[Abstract]:Objective to investigate the value of MRS technique in patients with non-hippocampal sclerosing temporal lobe epilepsy. Methods Twenty-three patients with unilateral temporal lobe epilepsy who were diagnosed and treated as non-hippocampal sclerosis in our hospital were retrospectively analyzed. Twenty normal volunteers with age matched were selected as the control group. Routine Mr plain scan and MRS were performed. To analyze the difference of N-acetylaspartate (NAA) / creatine (Cr) and choline chorionic acid (Cho) in the hippocampus of the patients with temporal lobe epilepsy, and to evaluate the pathological and operative results of 11 patients who received surgical treatment in the patients with temporal lobe epilepsy. Results the NAA/Cr value of the affected hippocampus in the case group was lower than that in the control group (P 0.001). The NAA/Cr value of the healthy hippocampus in the case group was not significantly different from that in the control group. There was no significant difference in hippocampal Cho/Cr between the control group and the control group. There was no significant difference between the two groups. The pathological results of 11 patients with epileptiform temporal lobectomy showed a small area of neuronal injury and no obvious glial hyperplasia. Conclusion MRS is helpful for clinical localization of epileptogenic foci in patients with non-hippocampal sclerosing temporal lobe epilepsy.
