【摘要】：目的通過(guò)研究股骨頭缺血性壞死在壞死指數(shù)與磁共振上的分期之間的相關(guān)性,找出壞死指數(shù)在股骨頭塌陷的臨界值,從而為利用MRI圖像預(yù)測(cè)股骨頭塌陷提供更可靠的參考標(biāo)準(zhǔn)(在股骨頭缺血壞死患者中,預(yù)測(cè)是否會(huì)發(fā)生股骨頭缺血壞死塌陷)。方法分析統(tǒng)計(jì)我院2012年6月至2014年2月明確診斷為股骨頭缺血性壞死且MRI檢查圖像可以進(jìn)行明確分期的共70例患者(共計(jì)100髖);依照倫諾克斯和亨格福德分期法(Lennox和Hungerford分期法)對(duì)100個(gè)股骨頭缺血性壞死的MRI圖像進(jìn)行分期,另按照Koo等提出的壞死指數(shù)法計(jì)算壞死指數(shù),對(duì)二者進(jìn)行統(tǒng)計(jì)學(xué)分析,評(píng)價(jià)其相關(guān)性,并確定股骨頭塌陷的臨界壞死指數(shù)值。結(jié)果 I期至Ⅳ期股骨頭缺血壞死指數(shù)分別為(0.31±0.08)(95%CI:0.29-0.34),(0.50±0.20)(95%CI:0.43-0.57),(0.62±0.18)(95%CI:0.53-0.71)及(0.83±0.23)(95%CI:0.70-0.95)。對(duì)各期股骨頭壞死指數(shù)進(jìn)行顯著性檢驗(yàn),結(jié)果顯示差異有統(tǒng)計(jì)學(xué)意義(P0.05)。以HSD法對(duì)任意兩期壞死指數(shù)進(jìn)行統(tǒng)計(jì)學(xué)分析,結(jié)果顯示兩兩之間差異均有統(tǒng)計(jì)學(xué)意義。Ⅱ期中有3髖壞死指數(shù)大于53%,Ⅲ期中全部壞死指數(shù)大于53%。結(jié)論壞死指數(shù)與股骨頭缺血壞死分期期數(shù)正相關(guān),統(tǒng)計(jì)分析得到壞死指數(shù)53%可視作預(yù)測(cè)股骨頭有無(wú)塌陷風(fēng)險(xiǎn)的臨界值,在臨床治療中,壞死指數(shù)可以為預(yù)測(cè)股骨頭缺血壞死是否塌陷提供參考。
[Abstract]:Objective to study the correlation between necrosis index and MRI staging of avascular necrosis of femoral head and find out the critical value of necrosis index in collapse of femoral head. This provides a more reliable reference standard for predicting femoral head collapse using MRI images (in patients with avascular necrosis of the femoral head, whether or not there will be avascular necrosis of the femoral head). Methods from June 2012 to February 2014, 70 patients (100 hips) diagnosed with avascular necrosis of femoral head in our hospital (total 100 hips) were diagnosed as avascular necrosis of the femoral head (100 hips) according to Lennox's and Hengerford's staging. MRI images of 100 avascular necrosis of femoral head were staging with Lennox and Hungerford staging. In addition, according to the necrotic index method proposed by Koo et al, the necrotic index was calculated, the correlation between them was analyzed statistically, and the critical necrosis index value of femoral head collapse was determined.
