3.0T心臟磁共振穩(wěn)態(tài)自由進(jìn)動(dòng)序列測(cè)量正常漢族中國(guó)人主動(dòng)脈根部:可行性、參考值范圍及影響因素

【摘要】:目的:采用3.0T CMR的穩(wěn)態(tài)自由進(jìn)動(dòng)序列(Steady-state free precession,SSFP)對(duì)正常漢族中國(guó)人主動(dòng)脈根部的形態(tài)學(xué)指標(biāo)進(jìn)行測(cè)量,評(píng)價(jià)其可行性,提出適合不同年齡段、不同性別正常漢族中國(guó)人的參考值范圍,并分析使得主動(dòng)脈根部各指標(biāo)變化的影響因素。方法:納入健康志愿者100名(20-70歲),分為5組(20~29歲組;30~39歲組;40~49歲組;50~59歲組;60~69歲組),每組男性和女性各10名。采用SSFP序列進(jìn)行3.0T CMR矢狀位(Sagittal)和冠狀位(Coronal)左室流出道(left ventricular outflow tract,LVOT)的電影圖像掃描,于最大收縮期和舒張末期層面分別測(cè)量主動(dòng)脈瓣環(huán)水平、主動(dòng)脈竇中部水平以及竇管交界水平的徑線值,比較各徑線值之間的差異。結(jié)果:共計(jì)掃描志愿者106人,最終納入100名志愿者,男女各50名,平均年齡為44.36±13.82歲。本研究測(cè)得了不同年齡段、不同性別正常漢族中國(guó)人的參考值范圍。收縮期與舒張期主動(dòng)脈根部各徑線測(cè)量值之間存在差異,如男性收縮期、舒張期在矢狀位瓣環(huán)水平的測(cè)量值分別為22.11±1.65 mm、21.37±1.84 mm,P0.05;女性收縮期、舒張期在矢狀位瓣環(huán)水平的測(cè)量值分別為20.87±1.45 mm、20.04±1.36 mm,P0.05。男性與女性主動(dòng)脈根部各徑線測(cè)量值之間存在差異,如男性和女性在矢狀位瓣環(huán)水平收縮期測(cè)量值分別為22.11±1.65 mm、20.87±1.45 mm,P0.05;舒張期相應(yīng)測(cè)量值分別為21.37±1.84 mm、20.04±1.36 mm,P0.05。冠狀位與矢狀位主動(dòng)脈根部各徑線測(cè)量值之間存在差異,如冠狀位和矢狀位瓣環(huán)水平男性的收縮期測(cè)量值分別為23.14±1.72 mm、22.11±1.65 mm,P0.05;舒張期相應(yīng)測(cè)量值分別為22.34±1.94 mm、21.37±1.84 mm,P0.05。不同年齡段主動(dòng)脈根部各徑線測(cè)量值之間存在差異,如20-29歲組、30-39歲組、40-49歲組、50-59歲組、60-69歲組矢狀位瓣環(huán)水平男性的收縮期測(cè)量值分別為21.24±1.28 mm、21.98±1.55 mm、22.11±1.44 mm、22.59±1.48 mm、22.61±1.33 mm,P0.05。不同水平主動(dòng)脈根部各徑線測(cè)量值之間存在差異,如瓣環(huán)水平、竇瓣結(jié)合部水平、主動(dòng)脈竇水平男性矢狀位收縮期測(cè)量值分別為22.11±1.65 mm、24.24±1.75 mm、30.81±1.94mm,P0.05。按照身高和體表面積標(biāo)化后,上述結(jié)果仍然成立(P0.05)。主動(dòng)脈根部各徑線值與年齡之間呈正相關(guān),隨著年齡的增加,主動(dòng)脈根部各徑線值亦增加(P0.05)。體表面積較身高對(duì)主動(dòng)脈根部各徑線值的影響更大,且對(duì)男性志愿者的舒張期徑線值的影響更大(P0.05)。結(jié)論:1.3.0T CMR的SSFP序列可用于正常漢族中國(guó)人主動(dòng)脈根部的評(píng)價(jià)。2.在對(duì)主動(dòng)脈根部進(jìn)行形態(tài)學(xué)評(píng)價(jià)時(shí),需要綜合考慮性別、年齡、身高、體表面積、心臟運(yùn)動(dòng)、不同徑線水平以及成像層面等因素對(duì)主動(dòng)脈根部各測(cè)量值的影響。
[Abstract]:Objective: to measure the morphological indexes of the aortic root of normal Han Chinese by using the Steady-state free precession (SSFP) sequence (Steady-state free precession, SSFP), to evaluate the feasibility of the aortic root of the Chinese Han people of the normal Han nationality, and to put forward the range of the reference value of the Chinese people of the normal Han people of different ages and different sexes, and to analyze the changes of the indexes of the root of the aorta. Methods: 100 healthy volunteers (20-70 years old) were divided into 5 groups (20~29 year old group, 30~39 year old group, 40~49 year old group, 50~59 year old group, 60~69 year old group), 10 men and women in each group. The film map of 3.0T CMR sagittal position (Sagittal) and coronary (Coronal) left ventricular outflow tract (left ventricular) was carried out by SSFP sequence. A total of 106 volunteers were enrolled in a total of 100 volunteers, 50 men and women, with an average age of 44.36 + 13.82 years. Conclusion: the SSFP sequence of 1.3.0T CMR can be used to evaluate the aortic root of the normal Han Chinese people when.2. is used for the morphological evaluation of the root of the aorta. The effects of sex, age, height, body surface area, heart movement, different diameter levels and imaging level on the measurement of aortic root values need to be considered comprehensively.
