【摘要】：目的探討阿爾茨海默病(AD)進程中,海馬頭、體、尾部形態(tài)學改變。方法對AD、輕度認知障礙(MCI)患者各30例(AD組、MCI組)及30名正常老年人(正常對照組)行顱腦MRI掃描�；贛RI掃描圖像將海馬頭、體、尾部進行分段,并測量其體積。比較AD組、MCI組、正常對照組海馬體積整體差異及頭、體、尾部體積差異。分析海馬體積與各神經(jīng)評定量表評分的相關(guān)性。結(jié)果 3組中,左側(cè)海馬總體積及頭、體、尾部體積均大于右側(cè)(P均0.05)。3組間左、右海馬總體積比較,AD組小于正常對照組(P均0.01)及MCI組(P均0.05),MCI組小于正常對照組(P均0.05)。與正常對照組比較,AD組左、右海馬頭、體、尾部體積均減小(P均0.05),MCI組左海馬頭、體部及右海馬頭部體積減小(P均0.05)。AD組左、右海馬頭、體部體積均小于MCI組(P均0.05)。簡易智能狀態(tài)檢查表(MMSE)評分與雙側(cè)海馬頭、體、尾部體積及總體積均呈正相關(guān)。除左海馬尾部外,雙側(cè)海馬頭、體、尾部體積及總體積均與日常生活能力量表(ADL)評分及臨床癡呆分級量表(CDR)評分呈負相關(guān),均與蒙特利爾認知評估量表(MoCA)評分呈正相關(guān)。結(jié)論 AD患者雙側(cè)海馬體積明顯縮小,MCI患者海馬體部、尾部萎縮不明顯,但海馬頭部體積明顯縮小。
[Abstract]:Objective to investigate the morphological changes of the head, body and tail of hippocampus in the process of Alzheimer's disease (AD).Methods craniocerebral MRI scans were performed in 30 patients with AD and 30 patients with mild cognitive impairment (MCI) and 30 normal elderly (normal control group).The head, body and tail of hippocampus were segmented and the volume was measured based on MRI scanning images.The differences of hippocampal volume, head, body and tail volume were compared between AD group and normal control group.The correlation between hippocampal volume and the scores of each nerve rating scale was analyzed.Results in the three groups, the total volume of left hippocampus, head, body and tail were all larger than that of the right group (P 0.05.3), the total volume of right hippocampus in AD group was lower than that in the normal control group (P 0.01), and the total volume of left hippocampus, head, body and tail of MCI group was lower than that of normal control group (P < 0.05).Compared with the normal control group, the volume of left and right hippocampal head, body and tail of AD group were all decreased (P < 0.05). The volume of body and right hippocampus head in 0.05).AD group was lower than that in MCI group (P < 0.05).MMSE scores were positively correlated with bilateral hippocampal head, body, tail volume and total volume.With the exception of left hippocampus tail, bilateral hippocampal head, body, tail volume and total volume were negatively correlated with ADL score and clinical dementia rating scale (CDRL), and were positively correlated with Montreal Cognitive Assessment scale (MMA).Conclusion bilateral hippocampal volume was significantly reduced in AD patients with MCI, and the tail atrophy was not obvious, but the hippocampal head volume was significantly reduced.
