MR動(dòng)態(tài)增強(qiáng)圖像紋理分析判斷乳腺結(jié)節(jié)良惡性的價(jià)值

【摘要】:目的探討MR動(dòng)態(tài)增強(qiáng)圖像紋理分析鑒別診斷乳腺結(jié)節(jié)良惡性的價(jià)值。方法回顧性分析經(jīng)手術(shù)病理證實(shí)的78例患者共80個(gè)乳腺結(jié)節(jié)的MR動(dòng)態(tài)增強(qiáng)圖像,每個(gè)結(jié)節(jié)獲得63個(gè)紋理特征參數(shù)。繪制紋理參數(shù)鑒別診斷良惡性乳腺結(jié)節(jié)的ROC曲線,并與MR乳腺影像報(bào)告和數(shù)據(jù)系統(tǒng)(BI-RADS)的診斷效能比較。結(jié)果 78例患者的80個(gè)乳腺結(jié)節(jié)中,紋理參數(shù)中灰度游程長(zhǎng)不均勻度判斷乳腺結(jié)節(jié)良惡性的AUC值(0.836)最大且診斷準(zhǔn)確率高,其診斷惡性乳腺結(jié)節(jié)的敏感度為82.93%(34/41)、特異度為94.87%(37/39)、準(zhǔn)確率為88.75%(71/80)、陽(yáng)性預(yù)測(cè)值為94.44%(34/36)、陰性預(yù)測(cè)值為84.09%(37/44)。MR BI-RADS分類診斷惡性乳腺結(jié)節(jié)的敏感度為95.12%(39/41)、特異度為87.18%(34/39)、準(zhǔn)確率為91.25%(73/80)、陽(yáng)性預(yù)測(cè)值為88.63%(39/44)、陰性預(yù)測(cè)值為94.44%(34/36)。MR BI-RADS分類和紋理分析判斷惡性乳腺結(jié)節(jié)準(zhǔn)確率差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.11)。與單獨(dú)應(yīng)用BI-RADS分類比較,兩者聯(lián)合應(yīng)用可明顯提高診斷惡性乳腺結(jié)節(jié)的特異度(P0.001)。結(jié)論MR紋理分析可作為傳統(tǒng)診斷乳腺良惡性結(jié)節(jié)的補(bǔ)充。
[Abstract]:Objective to evaluate the value of texture analysis in dynamic enhanced Mr imaging in differential diagnosis of benign and malignant breast nodules.Methods dynamic contrast-enhanced Mr images of 80 breast nodules in 78 patients confirmed by surgery and pathology were retrospectively analyzed. 63 texture parameters were obtained for each node.The ROC curve of texture parameters for differential diagnosis of benign and malignant breast nodules was drawn and compared with the diagnostic efficacy of Mr mammography report and data system BI-RADS.Results among the 80 breast nodules in 78 patients, the AUC value of gray run length heterogeneity in texture parameters was 0.836) and the diagnostic accuracy was high.The negative predictive value was 94.44%(34/36).MR BI-RADS classification and texture analysis. There was no significant difference in the accuracy of malignant breast nodules.Compared with the classification of BI-RADS alone, the combination of the two methods can significantly improve the specificity of the diagnosis of malignant breast nodules (P 0.001).Conclusion Mr texture analysis can be used as a supplement for traditional diagnosis of benign and malignant breast nodules.
