【摘要】：目的探討靜息態(tài)低頻振幅(ALFF)功能性磁共振及基于體素的形態(tài)學(xué)分析(VBM)在精神分裂癥腦功能與結(jié)構(gòu)變化中的應(yīng)用價(jià)值。方法符合入選標(biāo)準(zhǔn)的32例精神分裂癥患者(患者組)和32例健康志愿者(對(duì)照組)進(jìn)行靜息態(tài)下腦功能磁共振成像(Rs-f MRI)掃描及矢狀位薄層T1加權(quán)數(shù)據(jù)采集;采用Rest1.8軟件對(duì)患者組和對(duì)照組標(biāo)準(zhǔn)化后ALFF的腦圖進(jìn)行分析對(duì)比ALFF值差異;再通過(guò)對(duì)存在差異的ALFF異常腦區(qū)進(jìn)行患者組和對(duì)照組的VBM灰質(zhì)密度數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析灰質(zhì)密度圖像差異;最后結(jié)合ALFF及VBM結(jié)果,探討精神分裂癥患者腦功能及腦結(jié)構(gòu)的改變。結(jié)果與對(duì)照組相比,患者組ALFF值顯著增高(P0.05)的腦區(qū)是右側(cè)殼核、左側(cè)眶額回延伸至左側(cè)殼核和左側(cè)背外側(cè)額上回;ALFF值顯著降低(P0.05)的腦區(qū)是右側(cè)顳下回、左側(cè)扣帶回后部、左側(cè)角回。與對(duì)照組相比,患者組灰質(zhì)密度值減低(P0.05)的腦區(qū)是左側(cè)角回、右側(cè)楔前葉延伸至左側(cè)扣帶回后部、右側(cè)殼核、右側(cè)顳下回、左側(cè)眶額回延伸至左側(cè)殼核、右側(cè)內(nèi)側(cè)額上回;未發(fā)現(xiàn)患者組灰質(zhì)密度增高腦區(qū)。結(jié)論精神分裂癥患者在靜息態(tài)下存在較廣泛的異常活動(dòng)腦區(qū),且異�；顒�(dòng)腦區(qū)的灰質(zhì)密度普遍減低;該結(jié)果與精神分裂癥谷氨酸假說(shuō)中神經(jīng)元受到廣泛損傷,神經(jīng)遞質(zhì)循環(huán)回路失控理論較為一致。
[Abstract]:Objective to investigate the value of functional magnetic resonance imaging (fMRI) and morphologic analysis based on voxel (VBM) in the functional and structural changes of the brain in patients with schizophrenia.Methods 32 schizophrenic patients (patient group) and 32 healthy volunteers (control group) were examined with Rs-f MRI and T1-weighted sagittal data acquisition.Rest1.8 software was used to analyze and compare the ALFF values of ALFF between the patient group and the control group.Then the difference of gray matter density image of VBM in patients and control group was statistically analyzed by analyzing the difference of VBM gray matter density data in abnormal ALFF brain area. Finally, the changes of brain function and brain structure in schizophrenia patients were discussed by combining the results of ALFF and VBM.Results compared with the control group, the right putamen nucleus was the brain area in which the ALFF value was significantly increased (P 0.05). The left orbitofrontal gyrus extended to the left putamen nucleus and the left dorsolateral superior frontal gyrus decreased significantly (P 0.05). The right inferior temporal gyrus and the left posterior cingulate gyrus were the brain regions.Left corner gyrus.Compared with the control group, the left angle gyrus was the brain area in which the gray matter density decreased (P0.05). The right anterior cuneate lobe extended to the posterior part of the left cingulate gyrus, the right putamen nucleus, the right infratemporal gyrus, the left orbital frontal gyrus to the left putamen nucleus and the right medial superior frontal gyrus.No increase in gray matter density was found in the patient group.Conclusion there are a wide range of abnormal active brain areas in schizophrenic patients, and the density of gray matter in abnormal active brain regions is generally decreased, which is associated with the extensive injury of neurons in the schizophrenia glutamate hypothesis.The theory of neurotransmitter circulatory loop runaway is consistent.
