【摘要】：目的采用血管內(nèi)超聲彈性成像技術(shù)評(píng)價(jià)斑塊負(fù)荷(PB)與面積應(yīng)變(AS)的關(guān)系。方法對(duì)40只雄性純種新西蘭兔全程高脂飼料喂養(yǎng)。第2周末行腹主動(dòng)脈球囊拉傷術(shù)。第12周末行腹主動(dòng)脈血管內(nèi)超聲檢查,對(duì)每只動(dòng)物選擇兩個(gè)等回聲斑塊,采集不少于3個(gè)心動(dòng)周期的原位血管內(nèi)超聲圖像,選擇舒張末期連續(xù)2幀原位圖像,構(gòu)建血管內(nèi)超聲彈性圖。測(cè)量并計(jì)算血管外彈力膜面積(EEMarea)、血管腔面積(Lumenarea)、斑塊面積(PA)、PB、血管外彈力膜體積(EEMvolume)、血管腔體積(Lumenvolume)、斑塊體積(PV)、斑塊體積負(fù)荷(PVB)、斑塊最大厚度(Tmax)、斑塊最小厚度(Tmin)、斑塊偏心指數(shù)(EI)、血管重構(gòu)指數(shù)(RI)及AS。根據(jù)PB不同,將斑塊分為低負(fù)荷斑塊組(PB≤40%)和高負(fù)荷斑塊組(PB40%),并對(duì)兩組間以上參數(shù)的差異、斑塊形態(tài)學(xué)和力學(xué)參數(shù)的關(guān)系等進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果兩組的PA、PV、PB、PVB、Tmin、Tmax、Lumenarea、Lumenvolume、EI差異均有統(tǒng)計(jì)學(xué)意義(P均0.01)。經(jīng)逐步多元線性回歸分析,回歸方程為:Y∧=-6.921+10.430 X1+12.207 X2(Y∧:AS;X1:EI;X2:PB;R2=0.272,P0.001)。在矯正偏心指數(shù)對(duì)AS的影響后,低負(fù)荷斑塊組的AS明顯小于高負(fù)荷斑塊組(P=0.010)。結(jié)論動(dòng)脈粥樣硬化斑塊的負(fù)荷對(duì)斑塊力學(xué)穩(wěn)定性有影響。高負(fù)荷斑塊較低負(fù)荷斑塊更易受損。
[Abstract]:Objective to evaluate the relationship between plaque load and area strain (ASA) by intravascular ultrasound elastography.Methods Forty male pure New Zealand rabbits were fed with high fat diet.Abdominal aortic balloon tension was performed at the end of the second week.At the end of the 12th week, the abdominal aorta was examined by intravascular ultrasound. Two isoechoic plaques were selected for each animal. In situ intravascular ultrasound images of not less than 3 cardiac cycles were collected, and two consecutive in situ images at the end of diastole were selected.Intravascular ultrasound elastography was constructed.The index of EI, the index of vascular remodeling, the index of vascular remodeling, and the ASs.According to PB, plaques were divided into low load plaque group (PB 鈮，
