【摘要】：目的探討三維斑點追蹤成像(3D-STI)技術評價蒽環(huán)類藥物對彌漫大B細胞淋巴瘤(DLBCL)患者早期左心室功能損害的價值。方法選取2014年12月至2016年10月安徽醫(yī)科大學第一附屬醫(yī)院行蒽環(huán)類藥物化療的DLBCL患者28例,分別于化療前及化療2、4、6個周期(即累計劑量達100、200、300 mg/m2)后24~48 h內(nèi)行二維超聲心動圖及3D-STI檢查,獲取常規(guī)二維超聲心動圖數(shù)值及左心室整體面積應變(GAS)、整體縱向應變(GLS)、整體圓周應變(GCS)、整體徑向應變(GRS),并進行統(tǒng)計學分析。結果 GAS、GLS在化療4個周期[(-28.66±2.67)%,(-16.49±1.94)%]及6個周期[(-25.14±2.73)%,(-15.12±1.97)%]時,與化療前[(-34.86±2.90)%,(-18.36±2.32)%]及化療2個周期[(-34.05±2.09)%,(-17.75±1.91)%]時比較顯著降低,差異有統(tǒng)計學意義(P0.05)。Pearson相關分析顯示,GAS與多柔比星累計劑量呈負相關(r=-0.815,P0.05)。整個化療進程中,GCS、GRS無明顯變化。結論 GAS對DLBCL患者蒽環(huán)類藥物化療后出現(xiàn)早期左心室功損害的敏感性較高。3D-STI技術可以發(fā)現(xiàn)早期蒽環(huán)類藥物所致的心臟毒性。
[Abstract]:Objective to evaluate the effect of anthracycline on early left ventricular dysfunction in patients with diffuse large B cell lymphoma (DLBCLL) by three dimensional speckle tracking imaging (3D-STI).Methods from December 2014 to October 2016, 28 patients with DLBCL received anthracycline chemotherapy in the first affiliated Hospital of Anhui Medical University.Two dimensional echocardiography and 3D-STI were performed before chemotherapy and within 24 hours after chemotherapy for 6 cycles (i.e. cumulative dose of 100200300 mg / m ~ 2).The values of conventional two-dimensional echocardiography and the global area strain of left ventricle (GASA), the global longitudinal strain (GLSN), the global circumferential strain (GCS) and the global radial strain (GRS) were obtained and analyzed statistically.緇撴灉 GAS,GLS鍦ㄥ寲鐤，
