【摘要】：目的探討~1H-MRS在慢性血吸蟲肝病中的診斷價(jià)值。方法分別對(duì)經(jīng)臨床及病理證實(shí)的慢性血吸蟲肝病患者30例(肝病組)和健康志愿者38名(對(duì)照組)行肝臟常規(guī)MRI和~1H-MRS檢查,測(cè)量相關(guān)代謝物[水(H_2O)、膽堿(Cho)、脂質(zhì)(Lip)]的峰值及峰下面積,并進(jìn)行統(tǒng)計(jì)分析。結(jié)果慢性血吸蟲肝病組和對(duì)照組MRS圖像主要顯示為H_2O峰(4.6 ppm)、Cho峰(3.2 ppm)和Lip峰(1.3 ppm);慢性血吸蟲肝病組~1H-MRS H_2O峰、Lip峰峰值及峰下面積均低于對(duì)照組,且有統(tǒng)計(jì)學(xué)意義(P0.05);Cho峰峰值及峰下面積與對(duì)照組差異無統(tǒng)計(jì)學(xué)意義(P0.05)。受試者工作特征曲線(ROC)曲線下面積(AUC)從大到小依次為H_2O峰值、H_2O下面積、Lip峰下面積、Lip峰值,其AUC分別為0.805、0.761、0.729、0.668,特異度分別為0.600、0.733、0.670、0.633,敏感度分別為0.921、0.737、0.789、0.737;聯(lián)合得到MRS的AUC是0.861、特異度0.684、敏感度0.867。結(jié)論 ~1H-MRS對(duì)慢性血吸蟲肝病診斷有一定價(jià)值,其中H_2O峰值及峰下面積診斷效能最高。
[Abstract]:Objective to investigate the diagnostic value of 1 H-Mrs in chronic schistosomiasis liver disease.Methods liver routine MRI and 1H-MRS were performed in 30 patients with chronic schistosomiasis liver disease (liver disease group) and 38 healthy volunteers (control group).The peak value and subpeak area of related metabolites [H2OO, Cho, Lipin] were measured and analyzed statistically.Results the MRS images of chronic schistosomiasis liver disease group and control group mainly showed that the peak of H2O was 4.6 ppm-1. 2 ppm) and the peak of Lip was 1. 3 ppmm.The peak value and subpeak area of 1 H-MRS H2O peak in chronic schistosomiasis liver disease group were lower than those in control group.There was no significant difference between the control group and the control group in the peak value of Cho and the area under the peak.鍙楄瘯鑰呭伐浣滅壒寰佹洸綰，
