【摘要】：目的:通過(guò)觀察孕足月不同周期乳房按摩對(duì)宮頸成熟度、產(chǎn)程、泌乳的影響,探討乳房按摩的安全性與有效性,尋找對(duì)乳房進(jìn)行干預(yù)的合適時(shí)間。方法:將2015年12月—2016年12月來(lái)青海省人民醫(yī)院常規(guī)產(chǎn)檢的孕足月初產(chǎn)婦105例隨機(jī)分成2個(gè)觀察組和1個(gè)常規(guī)組共3個(gè)組(每組各35例)。常規(guī)組孕婦來(lái)我院進(jìn)行定期孕期的產(chǎn)檢、母乳宣教及進(jìn)行產(chǎn)后的常規(guī)護(hù)理,觀察組在常規(guī)組的基礎(chǔ)上施行由我院專人指導(dǎo)的乳房按摩,從孕37w開(kāi)始施行乳房按摩直至分娩為觀察1組,從孕38w開(kāi)始按摩直至分娩為觀察2組。觀察三組臨產(chǎn)后宮頸成熟度(bishop評(píng)分)、總產(chǎn)程、分娩孕周、自然宮縮情況、剖宮產(chǎn)率、產(chǎn)前血清泌乳素值、初次泌乳時(shí)間。結(jié)果:(1)三組孕婦在宮頸成熟方面的比較,臨產(chǎn)時(shí)的宮頸評(píng)分由高到低依次為觀察1組(8.14±1.76)、觀察2組(7.20±1.47)、常規(guī)組(5.66±1.60),各組組間比較有意義(P0.05)。(2)三組間的產(chǎn)程時(shí)間由短到長(zhǎng)的順序?yàn)橛^察1組(8.01±3.75)h、觀察2組(10.24±4.30)h、常規(guī)組(13.33±3.65)h,三組組間比較(P0.05),差異有顯著性。(3)三組分娩孕周比較,觀察組早于常規(guī)組(P0.05)。(4)三組發(fā)動(dòng)自然宮縮率的比較(P0.05),觀察1組(80%)高于常規(guī)組(51.4%),差異有顯著性。(5)三組剖宮產(chǎn)率及新生兒結(jié)局比較,差異無(wú)顯著性(P0.05)。(6)三組血清泌乳素值比較(P0.05),觀察1組(215.86±54.29)ng/ml、觀察2組(205.8±57.25)ng/ml均高于常規(guī)組(165.4±54.03)ng/ml(P0.05)。(7)初次泌乳時(shí)間比較(P0.05),觀察1組(8.29±3.64)h和觀察2組(8.93±3.98)h均比常規(guī)組(14.19±4.28)h短(P0.05)。結(jié)論:1.乳房按摩可促宮頸成熟、促進(jìn)發(fā)動(dòng)自然規(guī)律宮縮、縮短產(chǎn)程、促進(jìn)泌乳,安全、有效,值得推廣。2.妊娠37周開(kāi)始施行乳房按摩在促宮頸成熟、縮短產(chǎn)程方面效果更好,建議37周開(kāi)始干預(yù)。
[Abstract]:Objective: to investigate the safety and efficacy of breast massage in different cycles of pregnancy on cervical maturity, labor and lactation. Methods: from December 2015 to December 2016 in Qinghai Provincial people's Hospital, 105 cases of pregnant and parturient who underwent routine birth examination in Qinghai Provincial people's Hospital were randomly divided into 2 observation groups and 1 routine group (each group). 35 cases each. The routine group of pregnant women came to our hospital for regular pregnancy examination. Breast milk education and postpartum routine nursing were carried out. The observation group received breast massage directed by our hospital on the basis of the routine group, and the observation group 1 received breast massage from 37 weeks of pregnancy until delivery. From the 38th week of gestation to delivery, we observed the cervix maturation rate, the total labor process, the gestational weeks, the natural uterine contraction, the cesarean section rate and the serum prolactin value in the three groups. Results comparison of three groups of pregnant women in cervix maturation, The cervix score was 8.14 鹵1.76g in the first group, 7.20 鹵1.47 in the observation group, 5.66 鹵1.60 in the routine group. The order of labor process between the three groups was: observation group 1 8.01 鹵3.75 hours, observation group 10.24 鹵4.30hs, control group 13.33 鹵3.65hs, and control group 13.33 鹵3.65h. the order of labor process was as follows: observation group 1 (8.14 鹵1.76) h, observation group 2 (10.24 鹵4.30 min), control group (13.33 鹵3.65) h, and control group (13.33 鹵3.65) h. There was a significant difference among the three groups (P 0.05). Comparison of the rate of spontaneous uterine contraction in the three groups (P0.05, observation group 1) was higher than that in the routine group (51.4), the difference was significant (P < 0.05). The rate of cesarean section and neonatal outcome in the three groups were significantly higher than that in the control group. There was no significant difference in serum prolactin levels among the three groups (P 0.05, observation group 1: 215.86 鹵54.29 ng / ml, observation group 2, 205.8 鹵57.25)ng/ml, 165.4 鹵54.03 ng / ml, P 0.05.7)) the first lactation time of group 1 was 8.29 鹵3.64 h and the observation group 2 was 8.93 鹵3.98 h shorter than that of the control group (P 0.05). Conclusion Breast massage can promote cervical maturation. Promote the initiation of natural contractions, shorten the labor process, promote lactation, safe, effective, worth promoting .2.The beginning of 37 weeks of pregnancy breast massage in the promotion of cervical maturation, shorten the labor process is more effective, it is recommended to start intervention at 37 weeks.
