【摘要】：背景:髂骨是骨轉(zhuǎn)移瘤較常見(jiàn)發(fā)病部位,易穿破皮質(zhì)并形成軟組織腫塊。髂骨形態(tài)不規(guī)則,周?chē)Y(jié)構(gòu)復(fù)雜,在行骨水泥灌注時(shí)存在穿刺困難和骨水泥彌散情況難以判定等問(wèn)題。目的:比較CT引導(dǎo)和DSA引導(dǎo)下穿刺并行骨水泥灌注治療髂骨轉(zhuǎn)移瘤的臨床療效。方法:64例患者行髂骨骨水泥灌注治療,按治療方案分為2組,對(duì)照組32例應(yīng)用DSA監(jiān)視下穿刺灌注骨水泥,實(shí)驗(yàn)組32例應(yīng)用CT引導(dǎo)穿刺,術(shù)后行CT掃描觀察骨水泥彌散情況。統(tǒng)計(jì)并比較分析2組患者一次性穿刺成功率、骨水泥的滲漏率、疼痛目測(cè)類(lèi)比評(píng)分、Oswestry功能障礙指數(shù)。結(jié)果與結(jié)論:①2組患者均順利完成穿刺并骨水泥灌注,DSA引導(dǎo)組骨水泥外溢2例,CT引導(dǎo)組骨水泥外溢1例;②2組患者治療后的疼痛目測(cè)類(lèi)比評(píng)分、Oswestry功能障礙指數(shù)均較治療前改善(P0.05),2組組間比較差異無(wú)顯著性意義(P0.05);③2種方法緩解疼痛和恢復(fù)活動(dòng)的效果無(wú)差異;④結(jié)果表明,在注意骨水泥灌注時(shí)機(jī)后,CT引導(dǎo)下治療髂骨轉(zhuǎn)移瘤可取得滿(mǎn)意的效果。
[Abstract]:Background: iliac bone is a common site of bone metastases. It is easy to break through the cortex and form soft tissue masses. The iliac bone is irregular in shape and complex in surrounding structure. Objective: to compare the clinical effect of CT-guided and DSA guided puncture combined with bone cement perfusion in treating iliac bone metastases. The patients were treated with iliac bone cement infusion. According to the treatment plan, 32 cases in the control group were treated with DSA, 32 cases in the experimental group were treated with CT guided puncture, and 32 cases in the experimental group were treated with CT guided puncture. After operation, CT scanning was performed to observe the dispersion of bone cement, the success rate of single puncture and the leakage rate of bone cement were statistically analyzed and compared between the two groups. Results and conclusion all 12 groups of patients successfully completed puncture and bone cement perfusion with DSA guided group 2 cases of bone cement overflow in CT guided group 1 case of bone cement overflow 1 case of bone cement overflow 1 case of 22 patients after treatment. There was no significant difference in visual analogue score and Oswestry dysfunction index between the two groups. The results showed that there was no significant difference in pain relief and activity recovery between the two groups. The treatment of iliac bone metastases under the guidance of CT after paying attention to the time of bone cement perfusion can achieve satisfactory results.
