【摘要】：目的:探討MR擴(kuò)散張量成像(Diffusion Tensor Imaging,DTI)技術(shù)的各向異性分?jǐn)?shù)(Fractional Anisotropy,FA)以及表觀擴(kuò)散系數(shù)(Apparent Diffusion Coefficient,ADC)與腰骶叢脊神經(jīng)根受壓程度的關(guān)系,評(píng)價(jià)DTI技術(shù)在腰骶叢脊神經(jīng)根受壓程度中的診斷價(jià)值。方法:選取70例腰椎間盤(pán)突出患者行常規(guī)磁共振(Magnetic Resonance Imaging,MRI)及DTI檢查,在常規(guī)軸位T2WI上依據(jù)Lindblon分型法獲得L_5神經(jīng)根受壓程度的Lindblon分型(共4級(jí)),在DTI圖像上測(cè)量L_5神經(jīng)根的FA值、ADC值,分析Lindblon分型與FA值、ADC值的關(guān)系,并進(jìn)行雙側(cè)L3至S1神經(jīng)根擴(kuò)散張量纖維束成像(Diffusion Tensor Tractography,DTT)。結(jié)果:(1)患側(cè)L_5神經(jīng)根平均FA值顯著低于健側(cè)(t=-4.416,p0.01),患側(cè)L_5神經(jīng)根平均ADC值高于健側(cè)(t=3.334,p0.05);(2)各級(jí)神經(jīng)根平均FA值、ADC值的差異有高度統(tǒng)計(jì)學(xué)意義(F=50.347,P0.01;F=53.572,P0.01);(3)經(jīng)Spearman秩相關(guān)分析顯示神經(jīng)根受壓Lindblon分級(jí)與FA值呈負(fù)相關(guān)(rs=0.970,p0.01),與ADC值呈正相關(guān)(rs=-0.963,p0.01)。結(jié)論:(1)3.0T MRI擴(kuò)散張量和擴(kuò)散張量纖維束成像可以用來(lái)顯示腰骶部脊神經(jīng)根;(2)在3.0T MRI擴(kuò)散張量纖維束成像(DTT)圖上可以清楚、完整地顯示大部分患者的脊神經(jīng)根;(3)FA值、ADC值可以反映神經(jīng)根受壓程度,且隨著受壓程度的加重,FA值降低,ADC值升高。
[Abstract]:Objective: to investigate the relationship between anisotropic fractional fraction (Fractional fractional fraction) and apparent diffusion coefficient (Apparent Diffusion efficient) of diffused Tensor imaging (DTI) and the degree of compression of lumbosacral plexus spinal nerve root. To evaluate the diagnostic value of DTI technique in the degree of compression of lumbar and sacral plexus nerve root. Methods: 70 patients with lumbar disc herniation were selected for routine magnetic Resonance imaging and DTI examination. The Lindblon classification of the compression degree of L _ 5 nerve root was obtained by Lindblon classification on conventional T2WI. The FA value and ADC value of L _ 5 nerve root were measured on DTI images, and the relationship between Lindblon classification and FA value was analyzed. Bilateral L3 to S1 nerve root diffusion Zhang Liang fiber bundle imaging was performed on Diffusion Tensor tractography. Results the average FA value of L5 nerve root in affected side was significantly lower than that in healthy side, and the average ADC value of affected L5 nerve root was higher than that of healthy side t3.334 (p 0.05). The Spearman rank correlation analysis showed that the Lindblon grade of nerve root compression was negatively correlated with FA value and had a positive correlation with ADC value. Conclusion the diffused Zhang Liang of MRI diffusing Zhang Liang and the diffusive Zhang Liang fiber bundle imaging can be used to display lumbosacral sacral bundles. Spinal nerve root 2) can be clearly seen on 3.0T MRI diffusion Zhang Liang fiber bundle imaging. The results showed that the FA value and ADC value of most patients could reflect the degree of nerve root compression, and the ADC value increased with the increase of the compression degree.
