姿勢對腦卒中偏癱患者站-坐轉(zhuǎn)移下肢負(fù)重及穩(wěn)定性的影響

【摘要】:目的:探討不同的四肢體位對腦卒中偏癱患者站-坐轉(zhuǎn)移下肢負(fù)重及穩(wěn)定性的影響。方法:選取30例腦卒中偏癱患者為實(shí)驗(yàn)組,30例正常人為對照組,兩組受試者均在4種肢位下完成站-坐轉(zhuǎn)移,對受試者完成站-坐轉(zhuǎn)移的時(shí)間、雙下肢負(fù)重、人體重心點(diǎn)在冠狀面上的最大擺動幅度(COGX)進(jìn)行比較。結(jié)果:不同上肢體位下腦卒中偏癱患者站-坐轉(zhuǎn)移的所需時(shí)間、下肢負(fù)重及COGX的差異不顯著(0.05P0.1)。不同足位下腦卒中偏癱患者站-坐轉(zhuǎn)移的時(shí)間、下肢負(fù)重及COGX有差異,差異具有顯著性意義(P0.05);患足置后,腦卒中偏癱患者完成站-坐轉(zhuǎn)移時(shí)雙下肢負(fù)重的不對稱性要明顯小于健足置后(P0.05);而當(dāng)健足置后時(shí),腦卒中偏癱患者完成站-坐轉(zhuǎn)移的穩(wěn)定性要明顯優(yōu)于患足置后(P0.05)。正常人在不同四肢體位下其站-坐轉(zhuǎn)移的時(shí)間、下肢負(fù)重及COGX均無顯著性意義(P0.05)。結(jié)論:不同上肢體位對腦卒中偏癱患者站-坐轉(zhuǎn)移的穩(wěn)定性及下肢負(fù)重?zé)o明顯影響;不同足位能顯著影響腦卒中偏癱患者站-坐轉(zhuǎn)移時(shí)穩(wěn)定性及下肢負(fù)重,患足置后可視為一種潛在提高患側(cè)下肢功能的訓(xùn)練方法。
[Abstract]:Objective: to investigate the effects of different limb positions on the weight bearing and stability of the lower extremities of hemiplegia patients with stroke. Methods: 30 hemiplegic patients with stroke were selected as experimental group and 30 normal subjects as control group. The two groups of subjects completed station-sitting transfer in 4 different limb positions. The time of station-sitting transfer and the load of lower limbs were measured. Results: it took time for the patients with hemiplegic stroke in different upper limb positions to transfer from station to sitting. There was no significant difference in lower extremity load and COGX. There was significant difference in the time of station-sitting transfer, lower limb load and COGX in patients with hemiplegia under different foot positions (P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P 0. 05, P < 0. 05). In stroke hemiplegic patients, the asymmetry of lower extremity load was significantly smaller than that of P0.05 when the patients with hemiplegia had completed the station-sitting transfer, but when the healthy feet were transferred, the asymmetry of the lower extremity load was lower than that of the healthy feet. The stability of post-sitting transfer in stroke hemiplegic patients was significantly better than that in patients with foot placement (P 0.05). There was no significant difference in lower extremity load and COGX. Conclusion: different upper limb positions have no significant effect on the stability of station-sitting transfer and lower limb load in stroke hemiplegic patients. Different foot positions can significantly affect the stability of the patients with hemiplegic stroke during the standing-sitting transfer and lower limb load. The foot placement can be regarded as a potential training method to improve the lower extremity function of the affected side.
