急性胰腺炎患者健康教育效果評(píng)價(jià)指標(biāo)體系的構(gòu)建

【摘要】:目的初步構(gòu)建一套科學(xué)、系統(tǒng)、規(guī)范的急性胰腺炎患者健康教育效果評(píng)價(jià)指標(biāo)體系,為臨床護(hù)理人員實(shí)施系統(tǒng)、規(guī)范的健康教育及全面評(píng)價(jià)健康教育效果和質(zhì)量提供參考依據(jù)和有效指導(dǎo)。方法在文獻(xiàn)回顧、半結(jié)構(gòu)式專家訪談和理論分析的基礎(chǔ)上初步擬定急性胰腺炎患者健康教育效果評(píng)價(jià)指標(biāo)體系框架。通過德爾菲(Delphi)法對(duì)來自護(hù)理教育、護(hù)理管理和臨床護(hù)理三個(gè)領(lǐng)域的26名專家進(jìn)行正式的咨詢,并應(yīng)用百分權(quán)重法和乘積法對(duì)各級(jí)指標(biāo)的權(quán)重進(jìn)行賦值,使各級(jí)指標(biāo)得以量化。最后應(yīng)用EXCEL2011和SPSS22.0統(tǒng)計(jì)分析軟件對(duì)數(shù)據(jù)進(jìn)行收集、整理和分析。結(jié)果1.本研究經(jīng)過兩輪德爾菲咨詢最終形成急性胰腺炎健康教育效果評(píng)價(jià)指標(biāo)體系,其中包括健康知識(shí)、健康信念、健康行為、健康教育滿意度4個(gè)一級(jí)指標(biāo)和31個(gè)二級(jí)指標(biāo)。2.兩輪問卷回收率均為100%,表明專家積極性較高;兩輪德爾菲咨詢后,專家權(quán)威系數(shù)分別為0.84,表明專家具有較高的專業(yè)權(quán)威性;兩輪專家咨詢后,一、二級(jí)指標(biāo)的協(xié)調(diào)系數(shù)分別為0.203、0.327,表明專家組對(duì)各級(jí)指標(biāo)重要性意見基本協(xié)調(diào)一致,卡方檢驗(yàn)P0.01,差異有統(tǒng)計(jì)學(xué)意義。3.通過百分權(quán)重法和乘積法對(duì)各級(jí)指標(biāo)進(jìn)行權(quán)重賦值,健康知識(shí)、健康信念、健康行為4個(gè)一級(jí)指標(biāo)的權(quán)重分別為0.278、0.253、0.309、0.160。4.指標(biāo)內(nèi)容分析:第一輪專家咨詢根據(jù)研究設(shè)計(jì)的指標(biāo)取舍標(biāo)準(zhǔn)及專家意見建議,刪除“疾病的名稱”、“休息時(shí)的體位”、“患者對(duì)家屬的依賴”、“患者對(duì)醫(yī)護(hù)人員的依賴”、“翻身的重要性”、“灌腸與導(dǎo)瀉的目的”6項(xiàng)二級(jí)指標(biāo)條目,增加“急性胰腺炎的病情認(rèn)知”、“不同時(shí)期活動(dòng)要求及注意事項(xiàng)(急性期臥床休息目的;緩解期適量活動(dòng)的意義;術(shù)后活動(dòng)要求)”2項(xiàng)二級(jí)指標(biāo)條目,并將“飲食的要求”改為“不同階段的飲食要求(急性發(fā)作期禁食禁飲的目的;緩解期低脂清淡飲食的重要性;手術(shù)患者術(shù)前術(shù)后飲食要求)”,將“患者對(duì)家屬的依賴”和“患者對(duì)醫(yī)護(hù)人員的依賴”2個(gè)二級(jí)條目合并為“有尋求醫(yī)護(hù)人員和家屬幫助的意愿”。第二輪專家咨詢專家意見基本達(dá)成一致,最終確立了包含4個(gè)一級(jí)指標(biāo),31個(gè)二級(jí)指標(biāo)在內(nèi)的急性胰腺炎健康教育評(píng)價(jià)指標(biāo)體系。結(jié)論本研究在文獻(xiàn)回顧、理論分析、半結(jié)構(gòu)訪談和德爾菲咨詢的基礎(chǔ)上,最終形成包括4個(gè)一級(jí)指標(biāo)和31個(gè)二級(jí)指標(biāo)在內(nèi)的急性胰腺炎健康教育效果評(píng)價(jià)指標(biāo)體系,確定各級(jí)指標(biāo)的權(quán)重值,對(duì)各級(jí)指標(biāo)進(jìn)行量化。研究過程中,專家代表性好、積極性高、專家意見基本協(xié)調(diào)一致,整個(gè)研究過程嚴(yán)謹(jǐn),研究結(jié)果科學(xué)可靠,可以為臨床護(hù)士實(shí)施健康教育并評(píng)價(jià)其實(shí)施效果和質(zhì)量提供量化依據(jù),從而提高護(hù)士積極性,提高患者自我管理能力及健康教育質(zhì)量和效果。
[Abstract]:Objective to establish a scientific, systematic and standardized index system for evaluating the effect of health education in patients with acute pancreatitis. Standard health education and comprehensive evaluation of the effectiveness and quality of health education to provide reference and effective guidance. On the basis of semi-structured expert interview and theoretical analysis, the framework of evaluation index system of health education effect in patients with acute pancreatitis was preliminarily drawn up. Twenty-six experts in the three fields of nursing management and clinical nursing conducted formal consultation, and used the percentage weight method and the product method to assign the weights of the indicators at all levels. Finally, the data were collected, collated and analyzed by EXCEL2011 and SPSS22.0 statistical analysis software. Results 1. After two rounds of Delphi consultation, the evaluation index system of health education effect of acute pancreatitis was formed. These include health knowledge, health belief, healthy behavior, satisfaction with health education, and 31 secondary indicators. The recovery rate of the two rounds of questionnaires is 100, indicating that experts are highly motivated, and after two rounds of Delphi consultation, The expert authority coefficient is 0.84, which indicates that the expert has higher professional authority. After two rounds of expert consultation, the coordination coefficient of the first and second level indicators is 0.203 / 0.327, respectively, indicating that the expert group's views on the importance of indicators at all levels are basically coordinated and consistent. Chi square test P0.01, the difference is statistically significant. 3. Weight assignment, health knowledge, health belief through percentage weight method and product method. "Dietary requirements" should be changed to "Diet requirements at different stages (the purpose of fasting and drinking during acute attack, the importance of low-fat and light diet in remission period," and "Dietary requirements". "preoperative and postoperative dietary requirements of patients", combining "patients' dependence on family members" and "patients' dependence on health care personnel" into "willingness to seek assistance from health care personnel and family members". The agreement was basically reached. Finally, the evaluation index system of acute pancreatitis health education including 4 primary indexes and 31 secondary indexes was established. Conclusion this study is based on literature review, theoretical analysis, semi-structural interviews and Delphi consultation. Finally, an index system for evaluating the effect of health education in acute pancreatitis including 4 primary indexes and 31 secondary indexes was formed, and the weights of the indicators at all levels were determined, and the indexes at all levels were quantified. In the course of the research, the experts were well represented. The research results are scientific and reliable, which can provide quantitative basis for clinical nurses to carry out health education and evaluate the effect and quality of their implementation, so as to improve the enthusiasm of nurses.
