【摘要】：目的研究分析ICU重癥內(nèi)科患者感染鮑氏不動(dòng)桿菌的相關(guān)危險(xiǎn)因素。方法選取2013年10月-2015年10月醫(yī)院ICU重癥內(nèi)科病房患者469例為研究對象,統(tǒng)計(jì)鮑氏不動(dòng)桿菌感染率,對發(fā)生鮑氏不動(dòng)桿菌感染患者相關(guān)因素進(jìn)行單因素分析,并將單因素中顯著差異變量納入多因素logistic回歸分析,同時(shí)記錄28d患者死亡率。結(jié)果 ICU重癥內(nèi)科病房469例患者中,78例感染鮑氏不動(dòng)桿菌,感染率為16.6%;GCS評分、血清白蛋白、MBP、合并真菌感染、病程中出現(xiàn)休克、機(jī)械通氣7d與患者發(fā)生鮑氏不動(dòng)桿菌感染有關(guān);多因素分析顯示,血清白蛋白25g/L、合并真菌感染、病程中出現(xiàn)休克是患者發(fā)生鮑氏不動(dòng)桿菌感染的獨(dú)立危險(xiǎn)因素;28d后78例感染鮑氏不動(dòng)桿菌患者死亡33例,死亡率是42.3%。結(jié)論 ICU重癥患者鮑氏不動(dòng)桿菌感染率和死亡率高,血清白蛋白25g/L、合并真菌感染、病程中出現(xiàn)休克是患者發(fā)生鮑氏不動(dòng)桿菌感染的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to study and analyze the risk factors of Acinetobacter baumannii infection in patients with ICU. Methods from October 2013 to October 2015, 469 ICU patients were selected as the study subjects, and the infection rate of Acinetobacter baumannii was counted. Univariate analysis was performed on the related factors of patients with Acinetobacter baumannii infection, and significant differences in single factors were included in multivariate logistic regression analysis. Results 78 patients were infected with Acinetobacter baumannii, the infection rate was 16.6G CS score, serum albumin MBP, fungal infection, shock occurred in the course of the disease. Mechanical ventilation for 7 days was associated with Acinetobacter baumannii infection, multivariate analysis showed that serum albumin 25 g / L, complicated with fungal infection, Shock was an independent risk factor for Acinetobacter baumannii infection in patients with ICU. After 28 days, 33 patients died of Acinetobacter baumannii and the mortality rate was 42.3. Conclusion the infection rate and mortality rate of Acinetobacter baumannii in ICU patients is high. Serum albumin 25 g / L, accompanied by fungal infection and shock in the course of the disease, was an independent risk factor for Acinetobacter baumannii infection.
