【摘要】：目的對(duì)重癥監(jiān)護(hù)病房泛耐藥鮑曼不動(dòng)桿菌菌血癥的臨床特點(diǎn)及預(yù)后進(jìn)行總結(jié)。方法回顧性研究2012年1月至2015年12月31日北京大學(xué)第一院呼吸監(jiān)護(hù)室的鮑曼不動(dòng)桿菌菌血癥患者,收集患者人口學(xué)特點(diǎn)、臨床資料、3d及14 d的預(yù)后。結(jié)果納入8例患者,平均年齡(62.4±18.0)歲,男3例,女5例。所有患者均患有基礎(chǔ)疾病,6例為免疫抑制患者,7例患者發(fā)病前2周內(nèi)接受含有β內(nèi)酰胺酶抑制劑的復(fù)合制劑或碳青霉烯類抗菌藥物,6例接受機(jī)械通氣,肺臟是主要病源。菌種鑒定均為泛耐藥鮑曼不動(dòng)桿菌。患病48h內(nèi)平均急性生理與慢性健康狀況Ⅱ(APACHEⅡ)評(píng)分為(28.3±7.5)分,C反應(yīng)蛋白(18.2~231.0mg/L)和降鈣素原(0.1~25.0ng/ml)個(gè)體差異較大。3d死亡率為4/8,均為入APACHEⅡ25分的患者;14d死亡率為6/8,PCT0.5ng/ml的患者均死亡。結(jié)論泛耐藥鮑曼不動(dòng)桿菌菌血癥患者14d的預(yù)后和疾病嚴(yán)重程度及降鈣素原升高有關(guān)。對(duì)于APACHEⅡ25分、降鈣素原升高、之前接受機(jī)械通氣的高度可疑患者應(yīng)考慮在血培養(yǎng)結(jié)果回報(bào)前搶先治療。
[Abstract]:Objective to summarize the clinical characteristics and prognosis of pandrug-resistant Acinetobacter baumannii in intensive care unit (ICU). Methods A retrospective study was conducted on patients with Acinetobacter baumannii from January 2012 to December 31st 2015 in the Respiratory Care Unit of Peking University. The demographic characteristics of the patients and the prognosis of 3 days and 14 days of clinical data were collected. Results the mean age of 8 patients was 62.4 鹵18.0 years (male 3). All the patients had basic diseases. Six patients were immunosuppressive patients. Seven patients received mechanical ventilation with 尾 lactamase inhibitors or carbapenem antibiotics within 2 weeks before the onset of the disease.
