PI-RADS V2提高前列腺癌影像診斷效能的價(jià)值評(píng)估

【摘要】:目的探討前列腺影像報(bào)告和數(shù)據(jù)系統(tǒng)第2版(PI-RADS V2)提高前列腺癌影像診斷效能的應(yīng)用價(jià)值。方法初級(jí)、中級(jí)、高級(jí)職稱醫(yī)師各3名,根據(jù)各自經(jīng)驗(yàn)分別對(duì)符合入組標(biāo)準(zhǔn)的76例前列腺M(fèi)RI圖像進(jìn)行經(jīng)驗(yàn)評(píng)分,然后根據(jù)PI-RADS V2的評(píng)分標(biāo)準(zhǔn)進(jìn)行2次評(píng)分。分析2次PI-RADS V2評(píng)分及3個(gè)層次醫(yī)師PI-RADS V2評(píng)分的一致性。計(jì)算并比較3個(gè)層次醫(yī)師經(jīng)驗(yàn)評(píng)分與PI-RADS V2評(píng)分的相關(guān)性、經(jīng)驗(yàn)診斷與PI-RADS V2診斷結(jié)果的一致性,對(duì)比前列腺癌檢出的敏感度、特異度、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值,并繪制ROC曲線。結(jié)果 2次PI-RADS V2評(píng)分結(jié)果高度一致,3個(gè)層次醫(yī)師PIRADS V2評(píng)分具有較好一致性,3個(gè)層次醫(yī)師經(jīng)驗(yàn)診斷評(píng)分與PI-RADS V2評(píng)分正相關(guān)(r值分別為0.672、0.805、0.890)。3個(gè)層次醫(yī)師及PI-RADS V2診斷正確率呈遞增趨勢(shì),無效率呈遞減趨勢(shì),錯(cuò)誤率3個(gè)層次醫(yī)師呈遞減趨勢(shì),PI-RADS V2錯(cuò)誤率略高于高級(jí)醫(yī)師。ROC曲線結(jié)果顯示高級(jí)醫(yī)師的診斷準(zhǔn)確性最高,其次為PI-RADS V2。PI-RADS V2診斷敏感度最高,其次為高級(jí)醫(yī)師。高級(jí)醫(yī)師特異度最高,其次為PI-RADS V2。結(jié)論 PI-RADS V2具有良好的可重復(fù)性,對(duì)于中級(jí)、初級(jí)醫(yī)師,PI-RADS V2可以顯著提高其對(duì)前列腺癌影像診斷效能。
[Abstract]:Objective to explore the value of PI-RADS V2) in improving the diagnostic efficacy of prostate cancer. According to their own experiences, 76 cases of prostate MRI were evaluated according to the criteria of admission. Then according to the PI-RADS V2 scoring standard, the consistency of the two PI-RADS V2 scores and the PI-RADS V2 scores of the 3 levels of physicians was analyzed. The correlation between the three levels of physician experience scores and the PI-RADS V2 score was calculated and compared. The sensitivity, specificity, positive predictive value, negative predictive value of prostate cancer were compared between empirical diagnosis and PI-RADS V2 diagnosis. The ROC curve was drawn. Results the results of PI-RADS V2 scores were highly consistent twice, and the PIRADS V2 scores of 3 levels of physicians had good consistency. The positive correlation between the three levels of physicians' experience diagnosis score and PI-RADS V2 score was 0.672 / 0.805 / 0.890, respectively. The diagnostic accuracy of PI-RADS V2 and PI-RADS V2 showed an increasing trend. The failure rate showed a decreasing trend, and the error rate of PI-RADS V2 was slightly higher than that of the senior physician .ROC curve showed that the diagnostic accuracy of the senior physician was the highest, followed by the diagnosis sensitivity of PI-RADS V2.PI-RADS V2. The second was senior physician. The specificity of senior physician was the highest, the second was PI-RADS V2.Conclusion PI-RADS V2 has good reproducibility. For intermediate level, PI-RADS V2 can significantly improve the diagnostic efficacy of PI-RADS V2 in prostate cancer.
