【摘要】：目的探討剪切波彈性成像技術(shù)(SWE)對橋本甲狀腺炎(HT)不同分期的診斷價值。方法 104例經(jīng)臨床確診的HT患者,根據(jù)甲狀腺功能狀態(tài)分為HT甲亢組(26例)、HT甲功正常組(29例)、HT亞臨床甲減組(27例)和HT臨床甲減組(22例)。同期選擇健康志愿者作為健康對照組(50例)。利用SWE技術(shù)測量所有受檢者甲狀腺組織的楊氏模量值。采用Spearman相關(guān)分析法分析甲狀腺組織楊氏模量值與甲狀腺峽部厚度、甲狀腺微粒體抗體(TMAb)、甲狀腺球蛋白抗體(TGAb)滴度的相關(guān)性。結(jié)果健康對照組、HT甲亢組、HT甲功正常組、HT亞臨床甲減組、HT臨床甲減組甲狀腺組織楊氏模量值呈遞增趨勢(P0.01)。HT甲亢組剪切波彈性模量值低于HT甲功正常組、HT亞臨床甲減組、HT臨床甲減組,差異均有統(tǒng)計學意義(P均0.05)。HT甲功正常組剪切波彈性模量值低于HT亞臨床甲減組、HT臨床甲減組,差異有統(tǒng)計學意義(P均0.05)。結(jié)論剪切波彈性成像楊氏模量值與橋本甲狀腺炎不同分期患者臨床表現(xiàn)有一定相關(guān)性,提示超聲彈性成像對橋本甲狀腺炎病程分期有一定的診斷價值。
[Abstract]:Objective to evaluate the diagnostic value of shear wave elastic imaging (SWE) in different stages of Hashimoto's thyroiditis. According to thyroid function status, 26 cases of hyperthyroidism group were divided into 29 cases of normal hyperthyroidism group and 27 cases of HT subclinical hypothyroidism group (n = 27) and 22 cases of HT clinical hypothyroidism group (n = 22). At the same time, healthy volunteers were selected as healthy control group (n = 50). The results were measured by SWE technique. The Young's modulus of thyroid tissue and the thickness of thyroid isthmus were analyzed by Spearman correlation analysis. Results the titer of thyroid microsomal antibody TMAbn and thyroglobulin antibody TGAb. results the thyroid tissue Young's modulus of normal hyperthyroidism group and normal HT subclinical hypothyroidism group showed an increasing trend of thyroid tissue Young's modulus in HT clinical hypothyroidism group. The value of shear wave elastic modulus in hyperthyroidism group was lower than that in HT subclinical hypothyroidism group, while that in HT subclinical hypothyroidism group was lower than that in HT subclinical hypothyroidism group. The values of shear wave elastic modulus in normal HT group were lower than those in HT subclinical hypothyroidism group and HT clinical hypothyroidism group. Conclusion the Young's modulus of shear wave elastic imaging is correlated with the clinical manifestations of different stages of Hashimoto's thyroiditis, suggesting that ultrasonic elastic imaging has certain diagnostic value in the course of Hashimoto's thyroiditis.
