【摘要】：目的評(píng)價(jià)Carba NP試驗(yàn)用于產(chǎn)碳青霉烯酶腸桿菌科細(xì)菌和鮑氏不動(dòng)桿菌的應(yīng)用價(jià)值。方法將2014年-2015年臨床分離的耐碳青霉烯類抗菌素腸桿菌科細(xì)菌39株和鮑氏不動(dòng)桿菌37株,采用Carba NP試驗(yàn)檢測(cè)碳青霉烯酶;并采用PCR方法檢測(cè)碳青霉烯酶相關(guān)基因,評(píng)價(jià)該試驗(yàn)的靈敏度和特異性。結(jié)果 26株腸桿菌科細(xì)菌PCR檢出碳青霉烯酶相關(guān)基因,Carba NP試驗(yàn)檢測(cè)腸桿菌科的敏感性、特異性、陽性預(yù)測(cè)值與陰性預(yù)測(cè)值分別為96.1%、94.8%、92.5%和97.3%;21株鮑氏不動(dòng)桿菌PCR檢出碳青霉烯酶相關(guān)基因,Carba NP試驗(yàn)檢測(cè)鮑氏不動(dòng)桿菌的敏感性、特異性、陽性預(yù)測(cè)值與陰性預(yù)測(cè)值分別為4.7%、100.0%、100.0%和45.9%。結(jié)論Carba NP試驗(yàn)?zāi)芸焖贆z出攜帶KPC、NDM-1、IMP-1和VIM-2基因腸桿菌科細(xì)菌的碳青霉烯酶,未能檢出攜帶OXA-23基因的鮑氏不動(dòng)桿菌的碳青霉烯酶。
[Abstract]:Objective to evaluate the application value of Carba NP test for carbapenem producing Enterobacteriaceae and Acinetobacter baumannii. Methods 39 strains of Enterobacteriaceae and 37 strains of Acinetobacter baumannii were isolated from 2014 to 2015. Carba NP test was used to detect carbapenase, and PCR method was used to detect carbapenase related genes. The sensitivity and specificity of the test were evaluated. Results the sensitivity and specificity of PCR detection of carbapenase related gene in 26 strains of Enterobacteriaceae were detected by Carba-NP assay. The positive predictive value and negative predictive value were 92.5% and 97.3%, respectively. The sensitivity and specificity of the Carba-NP test for the detection of carbapenase related genes in 21 strains of Acinetobacter baumannii were detected by Carba NP test. The positive predictive value and the negative predictive value were 4.70% and 45.9%, respectively. Conclusion Carba NP test can quickly detect carbapenase of Acinetobacter baumannii carrying KPC-NDM-1IMP-1 and VIM-2 genes, but not of Acinetobacter baumannii carrying OXA-23 gene.
