【摘要】：目的:通過(guò)臨床研究,觀察針刺結(jié)合康復(fù)訓(xùn)練治療對(duì)腦卒中恢復(fù)期吞咽障礙患者吞咽功能的臨床療效,并探討表面肌電(Surface Electromyograp,sEMG)檢查在腦卒中吞咽障礙患者評(píng)估中的應(yīng)用價(jià)值。方法:采用洼田飲水試驗(yàn)篩選出90例腦卒中恢復(fù)期吞咽障礙患者,隨機(jī)分為三組,干預(yù)過(guò)程中脫落5例,故對(duì)最終85例受試者進(jìn)行分析,A組(28例):單純針刺組,B組(29例):常規(guī)康復(fù)訓(xùn)練組,C組(28例):針刺+常規(guī)康復(fù)訓(xùn)練組,在干預(yù)前和干預(yù)四周結(jié)束后各評(píng)定1次,各研究對(duì)象分別采用標(biāo)準(zhǔn)吞咽功能評(píng)價(jià)量表(SSA)和表面肌電檢查進(jìn)行評(píng)定,分析A、B、C三組的療效。結(jié)果:(1)在治療方面,三組在治療前差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05),具有可比性;三組治療后SSA及sEMG評(píng)分,差異具有統(tǒng)計(jì)學(xué)意義(p0.05),提示針刺結(jié)合常規(guī)康復(fù)訓(xùn)練在改善腦卒中恢復(fù)期吞咽障礙方面優(yōu)于單純針刺組或常規(guī)康復(fù)訓(xùn)練組;(2)在治療4周結(jié)束時(shí),三組患者的sEMG(平均波幅、峰值)和SSA得分較組內(nèi)治療前改善(p0.05),且治療后,針刺結(jié)合常規(guī)康復(fù)訓(xùn)練的sEMG和SSA得分優(yōu)于單純針刺組或常規(guī)康復(fù)訓(xùn)練組(p0.05)。結(jié)論:針刺結(jié)合常規(guī)康復(fù)訓(xùn)練能夠有效的改善腦卒中恢復(fù)期吞咽障礙患者的吞咽功能,且sEMG在腦卒中后吞咽障礙評(píng)定方面有一定應(yīng)用價(jià)值。
[Abstract]:Objective: to observe the clinical effect of acupuncture combined with rehabilitation training on swallowing function of patients with convalescent dysphagia after stroke. To explore the value of surface electromyograpstics EMG in evaluating the patients with stroke dysphagia, 90 patients with deglutition disorder were selected by depression-drinking water test, and were randomly divided into three groups. Therefore, the final 85 subjects were analyzed in group A (n = 28): acupuncture group (n = 29): group B (n = 29); group C (n = 28); group A (n = 28); group A (n = 28); group A (n = 28); group A: group A (n = 28); group C: group C (n = 28); All the subjects were evaluated by SSAs and EMG. Results: there was no significant difference before treatment among the three groups (P 0.05), and there was no significant difference between the three groups (P 0.05). The scores of SSA and sEMG in the three groups after treatment were significantly different (P 0.05). It suggested that acupuncture combined with routine rehabilitation training was superior to acupuncture group or routine rehabilitation training group in improving the dysphagia disorder in convalescent stage of stroke.) at the end of 4 weeks of treatment, acupuncture combined with routine rehabilitation training training group was better than acupuncture group in improving dysphagia disorder in convalescent stage of stroke. The sEMG (mean amplitude, peak) and SSA scores in the three groups were better than those in the pre-treatment group (P 0.05), and after treatment, The scores of sEMG and SSA of acupuncture combined with routine rehabilitation training were better than those of simple acupuncture group or routine rehabilitation training group. Conclusion: acupuncture combined with routine rehabilitation training can effectively improve the swallowing function of patients with convalescent dysphagia after stroke. SEMG is valuable in assessing dysphagia after stroke.
